Job summary
TheTransfer of Care System Leader is a system appointment. The post holder will work in partnership with the system health and care leaders to operationally deliver a multi-disciplinary and interdisciplinary approach to discharge from hospital based on the principles of no place like home, adopting a Home First approach.
The post will be hosted by one of the statutory partners and will report to the Assistant Director of Adult Social Care Operations for professional supervision and management. There will be dual reporting lines into the Trust and Council and the post holder will be situated as part of the management structure within both organisations. It is recognised that the delivery is on behalf of the system and will require full system support to achieve this.
This post is a partnership post and the expectation would be to interact alongside all system partners. In performing this system leadership role it is a requirement to develop a shared system view of transfer of care, to hold stakeholders to account, driving forward the actions that need to be taken as a system to address shared challenges.
Main duties of the job
The primary functions of this role are:
Strategic and operational oversight of the system wide Transfer of Care Hub, functions and policies.
The oversight and co-ordination of 'transfer of care' primarily hospital discharge arrangements and hospital admission avoidance for all people across the health and care system.
Escalating as necessary to the relevant Executive Leads for the Trust, ICB and Local Authority.
Implementation and system oversight of the Hospital discharge and community support policy

- Evidence of continuing professional development post graduate study.
- Health or Social Care related degree at Masters level or able to demonstrate substantial knowledge and experience within the relevant field with a willingness to work towards qualification.
- Advanced theoretical, practical & professional knowledge acquired through degree, supplemented by specialist training to postgraduate or equivalent level or experience, in relevant disciplines.
Desirable
- Professional registration within one of the key health or social care disciplines.
