Job summary
An exciting opportunity has arisen to recruit a motivated and enthusiastic Band 3 Health Care Assistant to work within the Community Nursing Team. As part of the Integrated Care Community (ICC) you will be predominantly based at Hilltop Heights within the Carlisle Network district nursing team, part of the developing Carlisle & District Team, supporting the local population. ICCs aim to address the challenges of an aging population and increasing demand on health and care services by providing more out of hospital care and empowering people to take control of their health and wellbeing. This will require new ways of working to focus on preventing ill health and ensure services are sustainable for the future.
This role requires you to demonstrate the skills and knowledge to support the provision of high quality care, to support patients with complex needs within the community setting.
We currently have a permanent 37.5 hrs per week (full time) position available. Daily working hours are between 8:00 and 23:00.

As a Health Care Assistant, working under the direction of a registered health care professional, undertake a range of delegated patient nursing and care duties including clinical observations and recording patient information. Working largely alone and unsupervised in the patients' home setting.
The Care Group is commissioned to provide 24/7 nursing services.
