Lead Care Co-ordinator Your Health Matters Team - 37.5 hours
This is an exiting opportunity to work within a newly established and innovative Occupational Therapy led proactive care team. The Your Health Matters Team identifies ' What is Important to you?' in managing your own health and wellbeing goals.
Based within the support of an established Rehabilitation team you will work closely, with primary and community care services, covering 7 GP practices within the beautiful Lake District National Park and Solway Coast.
We are looking for individuals who are passionate, enthusiastic and proactive in providing outstanding personalised care.
Main duties of the job
The role will provide Your Health Matters Team with all aspects of administrative support including the development of the service, co-ordinating patient's ongoing care and referrals to other services
Communicate courteously, effectively and consistently both with colleagues and callers to the service whilst retaining the ability to remain calm in difficult situations. To provide, receive and communicate complex or sensitive clinical information and process appropriately.
Utilise population health intelligence to proactively identify and work with a cohort of patients to deliver personalised care
To support the development and maintenance of systems which monitor internal spreadsheets to capture action plans and compliance against service targets.
Coordinate and support the team leads to ensure that all team members including this post have sufficient allocated time for learning and supervision and to have systems in place to manage and monitor all training and development.
Support the team to bring together all of a person's identified care and support needs, and explore options to meet these within a single personalised care and support plan.
Work with Primary Care professionals within the PCN to identify and manage a caseload of patients, and as required and appropriate, refer people back to other health professionals.
