We are looking for a physiotherapist who is forward thinking and passionate about patient care and rehabilitation. The post is based with the Copeland Rehabilitation Team, which includes the Western Lake District.

We are looking for someone who enjoys the stimulus of working in a team, and is happy to contribute ideas towards developing the service, but someone who can also be confident in their clinical judgement to work alone as required.

You will need to be experienced in managing patients with a variety of conditions, and be able to use your assessment and clinical judgement skills to the full. Ideally, you will also have some community experience.

Our team has a varied caseload, and patients are seen in their own homes (including residential and nursing homes); in community hospital beds or Interim Health beds; in group-based sessions or as individual outpatients.

The team works very closely with members of the MDT, as well as Adult Social Care and other agencies, and is part of the wider team of the Copeland ICC .

7 day working is required on a rota basis.

Main duties of the job

The main function of the Community Rehabilitation Team within the ICC is the prevention of unnecessary hospital admissions, facilitation of early discharge from the hospital setting and provision of patient centred support. This may be in the form of rehabilitation and/or care support plans.

You will be expected to be part of our Lead Professional of the Day rota, Discharge to Assess and contribute to the rapid response functions of the team.

The ICC also delivers group based rehabilitation, and provides input to the in-patients of Copeland Unit and the local health-funded Interim beds.

The post holder will use their professional knowledge and skills to support physiotherapy students on placement, and contribute to the ongoing learning and development of staff within the team and local MDT.

The successful applicant would be supported by a team of Occupational Therapists, Physiotherapists, Assistant Practitioners, Rehab Assistants, Home Care Practitioners and Hub coordination staff. We work very closely with the local GPs, District Nurses and Mental Health Team, as well as Adult Social Care and other agencies.

The core working hours of the service are Mon to Fri 8.30-16.30 but the post holder will be included on a rota to cover late shifts, weekend working and Bank Holidays. This is to support ICC working 7 days a week, 365 days a year 8am until 8pm.

As the post is a community post, a car driver is essential. A lease car scheme is available if required.
