Band 5 Occupational Therapist
An exciting opportunity has arisen for a Band 5 Occupational Therapist to work within the rehabilitation community therapy team at Keswick and Solway ICC. The post is full time, 37.5 hours per week which includes weekend working.
The role would suit an occupational therapist who already has some experience but would like to expand their knowledge and skills to cover community work. The post would be based in Keswick Hospital with the Community Therapy Team and would primarily cover Keswick Community Hospital inpatients as well as a focus on the confidence and balance service. There would also be the opportunity to participate in discharge to assess and rapid response activity.
We are a supportive team covering Keswick and Solway that consists of OTs, Physios, an HCP coordinator, APs, Hub administrators, Rehab Assistants and Home Care Practitioners. We work closely with the MDT including DNs, GP practices as well as many other community services, agencies and professionals.
The post holder will manage a defined caseload of patients, using evidence based principles to assess, plan, implement and evaluate interventions in hospital and community settings whilst facilitating safe and efficient discharge planning.
The post holder will also provide leadership and supervision to support staff and students within the team.
Main duties of the job
The Community Care Group provides a range of adult community health services to people living within Keswick and Solway ICC. Its key community health focus is to provide care to prevent hospital admissions, facilitate early discharge and where possible to enable patients to manage their own conditions and remain in their own homes, enhancing both their independence and quality of life.
The Community Rehab Teams in the Keswick and Solway ICC provide specialist assessment and rehabilitation for patients with a wide range of long and short-term conditions, working closely with our health and social care colleagues.
Patients are seen as inpatients, in their own homes (including residential and nursing homes) or as out-patients in the Rehab Centre. There is also a confidence and balance group that runs from Keswick Rehab Centre.
There is a strong focus on professional development opportunities and CPD within the team. You will have access to regular clinical supervision, and we offer an excellent period of induction, ongoing professional support and a highly rewarding job to the successful applicant.
The core working hours of the service are Mon to Fri 8.30-16.30 but the post holder will be included on a rota to cover late shifts, weekend working and bank holidays. This is to support ICC working 7 days a week, 365 days a year 8am until 8pm.
