Band 8a Qualified ACP 37.5 hours per week

We are looking to appoint a dynamic and forward-thinking Qualified Advanced Clinical Practitioner (ACP) to work within our urgent treatment centres based at Penrith Hospital with occasional cross site working at Keswick urgent treatment centre.

The post holder will be an experienced health care practitioner with leadership skills who will work autonomously in caring for patients with a range of urgent health problems whilst maintaining and developing their own advanced clinical assessment skills, diagnostic skills and understanding of investigations. They will manage full episodes of care including assessment of a range of systems, using clinical knowledge to formulate diagnosis, apply critical decision making in planning care/treatment plans and where appropriate, work collaboratively with medial and wider teams outside the UTC to meet the needs of patients, supporting the delivery of policy and procedure and using national and local guidelines to ensure care is based on sound evidence. Provide an environment supportive to learners of all levels.

Main duties of the job

  • Independently assess, differentially diagnose, plan, implement and evaluate individualised treatment plans for patients, resulting in safe appropriate management, referral or discharge without direct medical support on site.
  • Prescribe independently (where legally permitted) within scope of competence and within the Royal Pharmaceutical Society Competency Framework, national and locally adapted guidelines and best available evidence for a range of urgent/acute presentations appropriate to patients need and desired therapeutic outcomes.
  • Provide clinical care on priority of patient need and within the time constraints of the department opening hours including ensuring waiting targets are met. This entails managing patients who self-refer to the service and also through referral pathways from other teams such as NHS 111, GP, community pharmacy, community staff, other hospitals, CHOC etc.
