Cardiac Rehabilitation Specialist Clinical Lead, Band 7 37.5 hours per week

Develop and sustain a seamless service between primary, secondary and tertiary care for patients, and their families/ carers, who are affected by Coronary Heart Disease (CHD). Provide a link in the immediate post-discharge period acting as a source of advice and support for patients, families and primary and secondary health care professionals. Support, educate and develop the MDT concerning all aspects of cardiac rehabilitation. Advise and support colleagues in the voluntary/community sectors responsible for the phase 4 centres.

The cardiac rehabilitation department is responsible for the identification and assessment of people given the diagnosis of coronary heart disease (CHD) by medical consultant or General Practitioner. A holistic person-centred approach is used with patient and family/carer empowerment and excellent inter-professional communication being at the core.

Individual coronary heart disease risk factors are identified with patients and appropriate health targets agreed and advice given.

Emotional, psychological support and expert clinical advice is given to patients and families/carers and to primary and secondary care staff who are in contact with those affected by CHD. The department maintains the link between primary, secondary and tertiary care.

Main duties of the job

Duties and Responsibilities of the post include:

  • Provide clinical leadership, practice, support and advice to the MDT in the area of cardiac rehabilitation.
  • Take responsibility for the continuing assessment, planning, delivery, evaluation and development of the cardiac rehabilitation service.
  • Lead the delivery of the cardiac rehabilitation service on a day to day basis.
  • Identify, assess and plan individualised cardiac rehabilitation/recovery programmes for patients diagnosed with Ischaemic Heart Disease and Heart Failure.
  • To co-ordinate locally the implementation of standards for the management of cardiac rehabilitation patients.
  • Ensure the trust policies and guidelines relevant to the department are given due consideration.
  • Establish, evaluate and sustain standards of care through professional leadership.
  • Ensure and promote the safety of patients, staff and visitors to the department.
  • Support Matron in the delivery of corporate objectives
  • Ensure that all trust policies are adhered to at all times.
  • Lead the development of an audit culture,
  • Lead on health education for patients and their families/carers throughout the patient journey including the inpatient stay and within the out-patient exercise programme.

Work Setting and Review:

  • Work unsupervised undertaking organisation of workload and holding responsibility for the service.
  • A cardiology consultant will provide clinical support or advice as required.
