Job summary

Band 6 Clinical Nurse Specialist

A Respiratory Virtual Ward has been commissioned to provide a safe alternative to manage and monitor a specific group of patients with acute respiratory illness, as an alternative to hospital admission and /or to facilitate earlier discharge from hospital. The patients care will be delivered in their own home by utilising cutting edge patient friendly technology to provide remote monitoring. They will have regular telephone support and home visits as required.

The role of the virtual ward team is to coordinate the hospital based and community based elements of the ward by;

  • Promoting the benefits of the virtual ward to patients and HCP
  • Identifying and admitting patients to the virtual ward, ensuring seamless transfer from the hospital site to the virtual ward.
  • Remote monitoring and assessment of patients in the community
  • Delivering Respiratory expertise to ensure patients feel supported on the virtual ward
  • Employing holistic assessment and strategies to manage emerging health needs.
  • Provide administrative duties to facilitate the smooth operation of the service
  • To integrate into the wider Respiratory directorate and fulfilling duties in line with clinical base team 8 Clinical positions
  • 2 x WTE West Cumberland Hospital
  • 2 X WTE Cumberland Infirmary Carlisle
  • 4 X WTE North Cumbria Home Oxygen Service, Wigton Hospital with individual satellite bases

Main duties of the job

  • To be an integral member of the respiratory team with partnership working across the virtual ward MDT.
  • To be an acknowledged expert in the management of respiratory disease holding specialist knowledge, skills and experience
  • To be responsible for providing specialist advice and care to patients and their carers.
  • To hold delegated responsibility for the virtual ward patient caseload co-ordinating admissions and discharges for patients referred to the service using digital technology.
  • To assess and treat patients requiring respiratory care, including the provision of oxygen therapy if deemed appropriate within the virtual ward setting following clear care pathways supported by a named consultant.
  • Flexible seven day working is required to meet the needs of the patient population.
  • To ensure compliance with NMC/HCPC Guidelines and with Trust policies, procedures and protocols.
  • To provide leadership, mentorship and support to general practice teams developing agreed and specific clinical standards.
  • To maintain knowledge, skills, abilities and attitudes necessary to deliver an evidenced based service that is responsive to the changing needs of the locality population and the service.
  • To support other members of the respiratory teams as required and to deputise, as appropriate.
  • To represent the Trust and the speciality at local and National meetings related to the speciality.
