Band 6 Clinical Nurse Specialist up to 75 hours per week available based at Cumberland Infirmary
A virtual ward is a safe and efficient alternative to NHS inpatient care that is enabled by technology. Thousands of people across England are already benefitting from being treated at home on a virtual ward.
This innovative approach is delivering high quality care, safely and conveniently for people at home, often where they would rather be.
The purpose of this role is to work in collaboration with established specialist respiratory teams across NCIC. The Virtual Ward Clinical Specialist Nurse/ AHP will be a registered nurse or registered physiotherapist who has acquired additional specialist knowledge, skills and experience within the field.
The successful candidate(s) will work within a respiratory base team following existing pathways and methods of working to support the delivery of respiratory care for a caseload of patients.
In this role, there will also be a focus on identifying patients suitable for virtual ward admission using specified care pathways, and once identified using digital monitoring and applying care delivery as an alternative to hospital admission.
Successful applicants from these posts maybe required to move dept if needed due to winter funding requirements.
Main duties of the job
- To be an integral member of the respiratory team with partnership working across the virtual ward MDT.
- To be an acknowledged expert in the management of respiratory disease holding specialist knowledge, skills and experience
- To be responsible for providing specialist advice and care to patients and their carers.
- To hold delegated responsibility for the virtual ward patient caseload co-ordinating admissions and discharges for patients referred to the service using digital technology.
- To assess and treat patients requiring respiratory care, including the provision of oxygen therapy if deemed appropriate within the virtual ward setting following clear care pathways supported by a named consultant.
- Flexible seven day working is required to meet the needs of the patient population.
- To ensure compliance with NMC/HCPC Guidelines and with Trust policies, procedures and protocols.
- To provide leadership, mentorship and support to general practice teams developing agreed and specific clinical standards.
- To maintain knowledge, skills, abilities and attitudes necessary to deliver an evidenced based service that is responsive to the changing needs of the locality population and the service.
- To support other members of the respiratory teams as required and to deputise, as appropriate.
- To represent the Trust and the speciality at local and National meetings related to the speciality.
