Senior Occupational Therapist (or Band 5-6 Development Post)
An exciting opportunity has arisen for an enthusiastic, dynamic and highly motivated Band 6 Senior Occupational Therapist to join the Initial Assessment Service within Copeland ICC. It is a multi-disciplinary team consisting of District Nurses, Senior Staff Nurses, Physiotherapists, OTs and is also currently supporting an Apprentice Nurse. This post provides the opportunity to work within a developing, dynamic service at the western edge of the Lake District and offers ready access to one of the most beautiful areas of the UK.
ICCs aim to address the challenges of an aging population and increasing demand on health and care services by providing more out of hospital care and empowering people to take control of their health and wellbeing. This has required new ways of working to focus on preventing ill health and ensure services are sustainable for the future. These new ways of working follow the same approach across all eight ICCs within Cumbria, with the flexibility to tailor services to the needs of local people.
The service supports Discharge to Assess referrals, Urgent Community Response referrals, initial assessments and rehabilitation planning for patients new to our community caseloads or who have not been on a caseload for more than 6 months. It involves working closely with our health and social care colleagues within the integrated care community to ensure that cases are referred on to the right teams.
Main duties of the job
The role involves providing assessment and treatment to a varied caseload of patients in the community setting; this may be in the patient's own home or in CQC registered establishments. The patient is then handed over to the core teams for ongoing care and rehabilitation within 1-2 weeks.
The work will also focus on rapid response for the prevention of admission to hospital as well as supporting early discharge.
Applicants who do not yet meet all the essential criteria for this post as a Senior Occupational Therapist would be welcome to apply and be considered for a Band 5-6 development post. A structured development plan would be in place to support you develop in the role over a 12-18 month period. Progression to Band 6 Senior Occupational Therapist would be dependent on meeting the necessary criteria and competencies, a reasonable extension to the development period would be considered.
