Job summary

The post holder will be a member of the Camden Adolescent Intensive Support Service (CAISS).

The role of the Community CAMHS Practitioner will be to develop and provide CAMHS input to young people aged between 11-18yrs and their families. This role will be working with young people and their families who are finding it difficult to engagement with Tier 3 services, who are having a mental health crisis and who are at risk of Tier 4 admission, or are having a Tier 4 admission. This will involve intensive working with a variety of clients in community settings, within client's homes, foster placements, residential settings and within school settings. Providing in-reach to inpatient settings and developing functional relationships with colleagues in a range of agencies is also pivotal to this role.

Main duties of the job

Clinical

The post holder will assess, implement and evaluate individual care programmes and may take on the role of Care Co-ordinator for young people on their case load.

The post holder will be responsible for overseeing admissions of Camden patients to Tier 4 services. They will attend Care Programme Approach meetings and liaise between Tiers 3 & 4 to ensure a safe pathway and transition between tiers.

To establish and sustain effective links with other mental health workers, both in the community and hospital based services, thus facilitating continuity of care and co-ordination of services. To facilitate admission when indicated maintaining contact with the client and their family when in hospital and helping them prepare for discharge. To co-ordinate rehabilitative care packages, in conjunction with other professional agencies and networks, prior to discharge from inpatient units back to the community.

To carry out generic assessments of the mental health needs of young people within a therapeutic framework and to contribute fully to the care planning process. Assessment and treatment that is sensitive to individual ethnic and cultural backgrounds and needs of the service user. The aim of such intervention is to facilitate the young person's ability to function in the community and to promote the young person's competence and quality of life in a way sensitive to the young person's needs including their ethnic and cultural needs as they approach adulthood
