Job summary

We are looking for a highly motivated, enthusiastic and experienced nurse to join our Community Heart Failure CNS team in Tower Hamlets. This post is part time to cover Thursdays and Fridays.

The Heart Failure clinical nurse specialist is a role for an independent practitioner working within the multi-disciplinary cardiology team. The post holder will work closely with the whole multidisciplinary team to take responsibility for helping to coordinate the clinical care of heart failure patients within the borough.

Responsibilities will include education of patients and colleagues, coordination of patients' care between the GP, community and hospital including admission avoidance, systematic data collection, clinical audit and multidisciplinary team meeting attendance. The post holder will be expected to develop specialised skills and knowledge relating to the field and to perform nurse-led clinics and independent home visits.

Main duties of the job

To work with health professionals across all interfaces to improve the awareness and diagnosis of heart failure and to ensure appropriate systems that underpin good practice are in place. To achieve and maintain standard of care set by clinical guidelines (NSF for CHD and NICE guidelines) and protocols developed nationally and locally.

To liaise with MDT to ensure a model of care that will provide a comprehensive care package to all heart failure patients in acute care.

To provide access to care and support for patients through exercise and education programs and focus groups to enable them to optimise their potential and thereby improve and maintain the best possible quality of life.

Continuing to improve standards of patient care through evidence based research/practice and participation in the National Heart Failure audit.

Coordinate the provision of palliative care support for patients with end stage heart failure, promoting greater collaboration amongst the health care professionals caring for this group of patients. Co-ordinate care in the acute setting and ensure seamless care provided along the Heart Failure pathway.

To undertake nurse led Heart Failure clinics in the community as well as undertaking home visits, to manage patients symptoms and ensure that they are on optimal medications (titrate/initiate/discontinue as appropriate.)

Identify working groups to develop strategic guidelines and policies.
