Job summary

Due to the expansion of our current services, the Community Diabetes Team is seeking an experienced Diabetes Specialist Nurse to join our team. We are inviting applications for the above position to support the Tower Hamlets Community Diabetes Service, based at Mile End Hospital.

As part of a dedicated and enthusiastic team of Community Diabetes Nurse Specialists, you will work alongside other healthcare professionals, providing both clinical and educational support to patients with Type 1 and Type 2 diabetes, as well as their carers.The successful candidate will possess strong time management, organisational, and communication skills, with the ability to manage a clinical caseload and work autonomously. At Bart's Health Community Diabetes Service, we deliver structured education for Type 1 diabetes through the Dose Adjustment for Normal Eating (DAFNE) programme, and for Type 2 diabetes through the X-Pert educational programme. We also provide structured education for individuals at risk of developing diabetes (Pre-Diabetes Education). We are particularly interested in candidates who are passionate about advancing their expertise in diabetes technology.

Main duties of the job

The post is an exciting opportunity to work within the speciality of diabetes, contributing primarily to the delivery of the adult diabetes service at Mile End Diabetes Care Centre and within GP practices in Tower Hamlets.

The post holder will develop specialist skills enabling them to provide management plans, treatment advice, and support for adults with diabetes and their carers.

The post holder will be a clinical and educational resource for patients and health professionals; and will ensure a smooth transition from primary to secondary care occurs in terms of diabetes care.

The post holder will be a good team player always striving to improve the care of people with diabetes.
