Job summary

This post is a fixed term post with a subspecialty focus on Integrated Respiratory care. The appointee will work alongside 5 established respiratory consultants in a brand new, purpose-built respiratory unit. A dedicated team of respiratory nurses and physiotherapists oversee a 10 bedded Respiratory Virtual Ward. This post will facilitate an increase in this bed base, providing dedicated clinical oversight and strengthening the of the audit and governance of the current pathways. We are looking for an enthusiastic candidate who wishes to remove barriers between primary and secondary care.

The successful candidate will contribute to inpatient general respiratory care, and a 1 in 18 acute unselected medical rota. The respiratory rota is generously filled with trainees and innovative fellow roles (intervention and quality improvement fellowships). The Respiratory team have been highlighted for their high rates of satisfaction across the annual staff survey 2023.

The post holder will be expected to contribute to teaching undergraduate and postgraduate medical staff and other professional groups. They will support audit, clinical governance and improvement work happening across the department.

Main duties of the job

The successful applicant will join our dynamic respiratory department as as a substantive Consultant with a special interest in General Respiratory Medicine and Integrated care. The post includes 10 PA of activity. This is a Royal College of Physicians approved post.

This exciting and innovative post has been developed as there is increased recognition for Specialist Respiratory Support within our community services, to work collaboratively with the Trust, department and community partners to lead, develop and support patients with acute and chronic respiratory diseases.

The successful applicant will work closely with our multidisciplinary community respiratory team. This dedicated team already provides continuing care for patients with complex airways disease, chronic breathlessness and home oxygen assessment and review services (HOSAR).Collaborating with the existing team, the postholder would be driven to expand the implementation of the Community Virtual Wards Acute Respiratory Infection (CVW-ARI) across Newham, to provide a safe and efficient alternative to NHS bedded care and allow patients to receive the acute care they need in their own home. The CVW-ARI is expected to evolve from a respiratory early and supported discharge service to provide direct admission avoidance pathways through a process of direct community referrals, aiming for a full occupancy up to 20patients.

Job responsibilities

1. To contribute to the provision of a comprehensive and efficient acute and specialist Respiratory Medicine service at Barts Health NHS Trust.

2. To deliver excellence in care for patients admitted on acute unselected medical take.

3. To undertake a dedicated respiratory procedures list (bronchoscopy/pleural).

4. To work closely with colleagues in both primary and secondary care, to provide seamless, integrated care for patients with chronic respiratory conditions and acute exacerbations.

5. To provide evidence-based care for all inpatients and outpatients.

6. To develop community partnerships within the community virtual wards across north east London.

7. To have regard at all times for the clinical and quality standards.

8. To liaise effectively and on a timely basis with General Practitioners, community services, and all healthcare agencies, and to work collaboratively across North East London as required.

9. To maintain and promote team and multi-disciplinary work with the specialties working closely and regularly with the consultant medical staff.

10. To participate fully in the management of the Department and to liaise closely with the General Manager and the Clinical Director.

11. To take an active role in teaching and training in the department and Trust at both undergraduate and postgraduate level.

General Duties

1. Continuing responsibility for the care of patients in his/her charge, including all administrative duties associated with patient care.

2. Leadership, development and organisation of the services in his/her charge in line with the Trusts business plans. Liaison and communication with the Clinical Director of Respiratory Medicine, the General Manager and the Managing Director and the North East London Integrated Care Board.3. Development of his/her own interests and specialisms within the specialty and including active participation in research programmes.

4. Corporate and individual responsibility for the professional management of doctors in training including the approval and monitoring of on calls rotas and locum appointments.

Managerial arrangements

The post holder is responsible to the Clinical Director for Medicine. The post is covered by the National Health Service terms and conditions of service for Hospital Medical and Dental staff (England and Wales). The appointment is pensionable unless the appointee chooses to opt out of the National Health Service Pension Scheme.

On-going developments in the department are designed to enhance the care delivered to patients, with a commitment to the education of the trainee doctors.

Research and Quality Improvement

The post holder may have the opportunity to participate in departmental research, Quality Improvement projects focussed around the clinical and service development priorities of the department.All research proposals and projects must be processed through the Trusts Research and Development Office, and gain appropriate ethical clearance. Extensive research facilities exist at Barts Health NHS Trust. Research will be actively encouraged within the department and in conjunction with other departments of the hospital, St. Bartholomews and the Royal London School of Medicine and Dentistry and Queen Mary University of London, where there is a wide range of research activity.The post holder will be required to participate in all clinical and non-clinical governance activities related to the Respiratory medicine services, including mortality meetings.

Continuing Professional Development and Continuing Medical Education

The post holder is expected to take responsibility for their own Continuing Professional Development and take part in Continuing Medical Education activities, in line with the requirements of the Royal College of Physicians and in consultation with the Lead for Respiratory Medicine and the Clinical Director. The post holder will be expected to be committed to the concept of lifelong learning and produce and maintain a Personal Development Plan in agreement with their Clinical Director. Study leave is available as provided for under the Terms and Conditions of Service for Hospital Medical and Dental staff, subject to the approval of the Clinical Director/General Manager.

Teaching and Training

The post holder will be expected to participate fully in the postgraduate education of Respiratory Medicine StRs and Core Medical and Foundation trainees based at Barts Health NHS Trust, together with undergraduate medical, dental and nursing students, and will be expected to lead on certain aspects of teaching and training.

Shared office accommodation is located within the Respiratory Department with computers. 2024 will see the opening of a large state of the art Respiratory and Intensive care co-designed building. The comfortable and spacious office areas will enrich the working environment of the whole respiratory service.Secretarial support is provided and can be administered form the main hospital site and Shrewsbury road health centre.
