Applications are invited for the above posts to commence on a rolling basis from Feb 2019 for 6 months in the first instance.
The posts are intended to provide critical care experience in an acute adult critical care unit providing both HDU and ITU level care, to anaesthetic and non-anaesthetic trainees who will have the opportunity to participate in educational activities, training, audit and research within the department of Critical Care Medicine. You would join a team that includes over 30 trainees and 17 Consultants.
The Royal London Hospital (RLH) forms part of Barts and The London NHS Trust. The ACCU (Adult Critical Care Unit ) is a 44 bedded critical care unit in the brand new hospital into which the Royal London moved in December 2011. The ACCU accepts referrals for medical and surgical patients requiring level 2 and 3 care. The hospital receives tertiary referrals for a number of Medical Specialties including Neurology, Gastroenterology, Hepatology, Renal and Stroke Medicine, as well as surgical specialties such as hepatobiliary surgery, upper and lower GI tract surgery, major vascular, maxillofacial and ENT surgery.
The RLH is the base hospital for the London's Helicopter Emergency Medical Service (HEMS) and the NE London Stroke Referral Unit. The Accident & Emergency Department caters for over 100,000 patients every year.
Applicants MUST have current GMC Registration and a minimum of 2 years clinical experience post qualification.
Main duties of the job
These posts are for Clinical Fellows in the ACCU, the new 44 bedded Adult Critical Care Unit at The Royal London Hospital. The Critical Care unit has 44 beds, of which 22 are dedicated level 2 beds accepting both surgical and medical patients and 22 are level 3 beds accepting a range of trauma, neuro and medical patients. There is also a separate 15 bedded post surgical recovery unit which is also covered by the ACCU medical team and Anaesthesia. The unit is staffed by a team of 19 Consultant Intensivists and 40 trainees from a variety of specialties and interests (Intensive Care medicine, Anaesthesia, Acute Medicine, Respiratory medicine, Surgery, Research Fellows). These Clinical Fellow posts are part of the ACCU trainee rotation, covering both level 2 and level 3 patients.

Patients with trauma or neurotrauma make up over 35% of admissions to the ACCU. These patients often present with a complex range of injuries which provide many challenges to care. We admit many patients with isolated neurotrauma, or with head and torso injuries, requiring neurocritical care.

- Admitting emergency patients to the ACCU.
- Involvement in day-to-day inpatient management under the direction of the Consultants and other senior trainees.
- Liaising with medical / surgical teams about patients admission, progress and discharge.
- Liaising with nursing staff, physiotherapists and other clinical staff about patient status.
- Caring for patients on the ACCU. This includes prescribing and supervising treatment, instituting invasive monitoring and gathering and interpreting results.
- Explaining the patients condition and progress clearly to patients and relatives.
- Maintaining accurate medical records, including data for audit purposes.
- Providing and receiving pertinent handover at the start and at the end of shift.
- Participation in educational activities including formal teaching sessions, X-ray meetings, journal clubs and case presentations.
- Development of his/her own interests, including active participation in clinical audit.
- On- call will be a full-shift which is consistent with 1A banding under the New Deal on junior doctors hours.
- Reviewing patients referred to the critical care service from other wards and from the A and E Dept
