Advanced Nurse Practitioner in Primary Care

Job summary

The post holder will act autonomously within their professional scope of practice,will demonstrate critical thinking, is a caring, compassionate and committed experienced nurse who, acting within their professional boundaries. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.

They will communicate and work collaboratively with the Primary Care Network and General Practice Teams to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

Main duties of the job

Assess, plan, develop, implement and evaluate treatment programmes that promote health and well-being, to be delivered both the GP Practice and Patients Home.

Confidently manage long term conditions within the scope of general practice - such as diabetes, asthma, chronic obstructive pulmonary disease and hypertension in line with the local/national quality outcomes framework, NICE and other relevant clinical guidelines. To include assessment, planning and implement and evaluate individual treatment plans for patients with a known long-term condition.

Be confident independently prescribing and reviewing medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evident practice and national and practice protocols

Proactively identify, diagnose and manage treatment plans for patents at risk of developing long term conditions as appropriate

Work with patients to support adherence to prescribed treatments

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations including initiation of effective emergency care

Support patients to adopt health promotion strategies that promote patients to live healthy, and apply principles of self-care

Triage visits and provide telephone advice as appropriate
