Job summary
The post holder will be responsible for implementing the National Policy and Framework for NHS Continuing Healthcare, NHS Funded Nursing Care and Personal Health Budgets for Continuing Healthcare clients on behalf of the DGFT, taking into account the DOH Responsible Commissioner Guidance.
To take the lead role in service development and redesign based on analysis of CHC Service provision together with the ability to drive the service forward. To be responsible for the planning, monitoring and reviewing the continuing care service, providing expert clinical leadership and advise to the continuing healthcare team and wider health and social care economy including community and acute providers and Local Authority
Expected to work autonomously and use own initiative in developing and innovating continuing healthcare services.
Ensuring systems are in place to enable to timely decision making around CHC eligibility and fast track decision and to organise and maintain the momentum of weekly continuing healthcare panels for adults, with appropriate professional representation on panels.
Responsible to ensuring the CHC function is fully supported with up to date standard operating procedures and system policies in line with national directives and organisational instruction. Work hand in hand with the DGFT's safeguarding team to address the safety of patients in commissioned placements.
Main duties of the job
To develop and maintain systems and processes that build on the work of the CHC Team.
To maintain an awareness of legislation and changes to policy guidance affecting NHS Continuing healthcare and Funded Nursing Care.
To support the smooth running of the continuing healthcare and PHB Panels, providing a consistent approach to decision in accordance with the National Framework.
To manage the continuing health care team and PHB panel process to ensure it is efficient, effective, transparent and equitable. Ensuring clients and their representatives have the opportunity to present their case.
To attend monthly meetings with NHSE for CHC/ PHB leads cascading information to the rest of the team and commissioning managers.
To work in accordance with the NMC code of professional conduct maintaining standards of care of current local and national best practice legislation.
To keep up to date with current knowledge and advances in relevant health and social care practice and maintain a clinical creditability ensure that practice is evidence based.
Be responsible for own continuing professional development and interpret appropriate professional clinical guidelines and standards in line with NMS registration and patient safety requirements.
