Job summary
An excellent opportunity has arisen for a highly motivated, enthusiastic and caring person to join our Community Nursing Team within Dudley NHS Foundation Trust, as part of the Coseley, Sedgley & Gornal team based at Ladies Walk. You will be part of a team providing high quality nursing care to patients in the community, driver essential. Minimum training requirements are a qualification in NVQ Level 3 Health and Social Care.
Previous experience as a Health Care Assistant in Acute or Community Nursing setting essential. You will have good communication skills and be able to work independently and as well as part of a team.
Main duties of the job
The post-holder is responsible for the ongoing assessment of care needs and the development, implementation and evaluation of programmes of care. The post holder will carry out all relevant forms of care and will take responsibility for caseload holders in their absence.
