Job summary
We are looking for a highly self-motivated and enthusiastic individual with exceptional administration and IT skills to join our team of Care Co-ordinators.
To act as a care coordinator in RHH, liaising closely with the CPT care coordinators who support 47 GP practices in Dudley. The post holder will be part of the front door team and work closely with the hospital discharge team, to support and facilitate safe discharges from ED, SDEC, AMU and the wards, liaising with the CPT care coordinators and community services as required.
Main duties of the job
- To monitor ED boards, identifying patients who may need care coordination intervention or support
- Communicate effectively and sensitively with patients and their carers, understanding what services they already have and explaining services that might be suitable, gaining consent for referrals as necessary.
- To act as a care coordinator in RHH, liaising closely and effectively with the CPT care coordinators
- The post holder will be part of the front door team and work closely with the hospital discharge team, to support and facilitate safe discharges from ED, SDEC, AMU and the wards,
- To have a good understanding of community services, liaising with services and making referrals as required.
- Provide complex information to patients, their carer's and/or relatives on behalf of the
- Be the point of liaison for CPT care coordinators when patients attend ED or are admitted
- Work closely with the High intensity User Teams and attend meetings as required, gather information as necessary prior to the meetings
