The successful applicant would be able toidentifyand address the health inequalities experienced by people with a learningdisability,work in a multiprofessional environment to affect change,be innovative and able to workautonomouslyto ensure reasonable adjustments are made todeliver safe andappropriate care.

Main duties of the job

Support people with learning disabilities, their families and carers in preparing the person for planned hospital admission and during emergency admission.

To act as a resource, disseminate information and offer advice and guidance for all staff involved in the care of people with learning disabilities.

To actively assist people with learning disabilities, as well as carers and health and social staff when using acute services.

To provide support and advice regarding the use and application of the Mental Capacity Act 2005, best interest and consent to the service user, their family/carer, and the professionals involved in decision making about their care.

Share knowledge and skills to hospital staff on the needs of patients with learning disabilities, for example: Mental Capacity Act / consent, reasonable adjustments, epilepsy, challenging behaviour, mental health, autism and communication.
