Patient Safety and Risk Midwife

Job summary

We are seeking a passionate and experienced midwife to lead on patient safety and risk within our maternity services. This pivotal role supports the delivery of high-quality, safe care by embedding a culture of learning, transparency, and continuous improvement.

As the Patient Safety and Risk Midwife, you will be responsible for overseeing incident management, supporting investigations under the Patient Safety Incident Response Framework (PSIRF), and ensuring timely dissemination of learning. You will work closely with clinical teams, Quality and Safety Leads, and board-level safety champions to triangulate safety intelligence and drive meaningful change.

Main duties of the job

  • The post holder will have a patient centred approach and will work in such a way as to help promote an open and transparent culture that actively improves clinical safety and quality.

  • The post holder will actively support the Clinical Governance Midwifery Manager in the maintenance of the Clinical Governance agenda within the Maternity Service.

  • The post holder will be responsible for the management of incidents on the Datix reporting system in the absence of the Risk/IT Midwife under the direction of the Clinical Governance Midwife. They will assist the Risk/IT Midwife in producing a monthly incidence report of all incidents received via the Datix system.

  • The post holder will attend Directorate/Board/external meetings in the absence of the Clinical Governance Midwifery Manager and update/present cases as required. The post holder will also attend debrief meetings as required.

1. Risk Management: to include level of responsibility for:

1.1 Actively support the Clinical Governance Midwifery Manager in the maintenance of the Clinical Governance agenda within the Maternity Service and be an active member within the clinical risk management systems. This will include the assisting with risk assessments for Maternity Services.

1.2 Support all Senior Midwives in the close monitoring of the Datix incident reporting system.

1.3 Assist with producing a monthly incidence report of all incidents received via the Datix incident reporting system.

1.4 Undertake in-depth, timely, case reviews, root cause analysis and patient safety incident investigations. Discuss and present as required at various Directorate/Board meetings.

1.5 Attend patient debrief meetings as required with a lead obstetrician/neonatologist and provide support/advocacy for families.

1.6 Monitor complaints in the absence of the Clinical Governance Midwifery Manager

1.7 Report cases to HSIB/MBRRACE/CDR agencies where appropriate

1.8 Facilitate / attend pertinent meetings in the absence of the Clinical Governance Midwifery Manager.

2. Clinical responsibilities: to include level of responsibility for

2.1 Maintain personal contact with patients and meets their needs on an individual basis, using the process of listening, assessing, planning, implementing and evaluating care.

2.2 Deal with the anxieties of relatives and arranges the support they may need.

2.3 Report any untoward incidents. Maintain security.

2.4 Maintain the supply and custody of drugs, medicines etc, including keeping of agreed register.

2.5 Ensure that measuring, administration and recording of all drugs and medicines is carried out in accordance with the rules of the NMC, and as laid down in Trust policy.

2.6 Support accurate and contemporaneous records of all care by all staff.

2.7 Liaise with all other personnel regarding access to the department, promoting relationships to enhance the quality of care to the consumer and ensuring effective communication with health care professionals.

3. Managerial responsibilities: to include level of responsibility for:

3.1 Participate in regular meetings of ward / unit staff, attends as a representative at other meetings when required.

3.2 Chair meetings and working groups as appropriate.

3.3 Deputise for the Clinical Governance Midwifery Manager in her absence

3.4 Maintain a safe working environment in accordance with the Health and Safety at Work Act, etc and local policies. Comply with the departments fire regulations. Ensure the proper reporting of all accidents / incidents. Participate in investigations of any complaints. Understands and carries out NHS Trust Policies as contained in Ward / Unit.

3.5 Ensure that directly managed staff are kept up to date and comply with Trust and local policies and procedures.

3.6 Participate in the development of Clinical Governance initiatives in conjunction with the Clinical Governance Midwifery Manager and monitor implementation and performance.

4. Professional responsibilities: to include level of responsibility for:

4.1 Provide a role model for others to emulate, at all times exhibiting high standards of professional dress and behaviour and ensuring effective communication with all members of the multidisciplinary team, patients, carers and relatives.

4.2 Contribute to professional forums where appropriate to job role and support professional growth and development of self and others.

4.3 Maintain knowledge and skills through a personal development plan agreed with line manager.

4.4 Work within the professional role governed by The Code of Practice/Rules of the NMC, as well as locally agreed policies and procedures.

5. Education and development responsibilities:
