Job summary

The enhanced care team offer additional support across all the wards and departments for patients who may require additional support due to, unfamiliar surroundings, medication, mental health issues or challenging behavioural needs.

Enhanced care is an integral part of a therapeutic plan and ensures the safe and sensitive monitoring of a patients physical and psychological wellbeing, whilst building therapeutic relationships. Enhanced care could be supporting 'cohorted' or patients who require 1-1 support.

Main duties of the job

They post holder be responsible for the effective and efficient operational management of the enhanced care team. The enhanced care team leader will be responsible for working within the set budget, clinical governance, and staff development and training.The post holder will be expected to provide professional leadership within the clinical area and provide direction, and inspirational leadership to all members of the multi-disciplinary team working with the enhanced care team. The post holder will be involved in the development of protocols / guidelines and patient information.

They will work collaboratively with ward teams and the Liaison Psychiatry Service to ensure patients receive effective, high quality clinical support to enhance mental health care, in accordance with the principles of least restriction.

The post holder will be responsible for ensuring that there are systems and processes for the planning, provision and review of enhanced care. The post holder will ensure that the enhanced care team deliver therapeutic engagement and interventions to patients with mental health conditions, cognitive impairment or those experiencing psychological distress.

They will provide regular reports on the effectiveness of the team in delivering quality care by reducing restrictive practices and incidents of harm.

As a clinical leader provide support to the management team, participating in service and policy development.

Act as an effective role model and resource/advisor to colleagues, in the delivery of nursing care.

Work autonomously within the parameters of the role, directing, leading and supporting junior staff and wider care team to implement the delivery of quality nursing care, contributing to physical, psychological, social, recreational and spiritual care in a recovery-focused way through therapeutic engagement and activities, and taking steps to address any issues that arise.

Build and support colleagues to build positive therapeutic relationships with patients needing Enhanced Care Supervision ensuring that their physical and psychological needs are met.

Manage and oversee the deployment of the Enhanced Care Team in the delivery of enhanced observations to inpatient service users and those waiting in A&E. Prioritising resources in line with patient need and the skills, experience and competency required to deliver enhanced care.

Provide practical support and guidance (in collaboration with elderly care and mental health colleagues) to clinical teams

Manage patients with complex clinical and mental health conditions that pose a risk in maintaining a conducive and safe environment in which clinical and nursing care can be delivered.

Ensuring that patients are provided with accurate information about care in an accessible format to patients and their relatives / carers

  • Current professional registration as an RMN/AHP/Social Worker with extensive experience of managing a caseload of patients.
  • Educated to a degree level or equivalent experience in Healthcare service.
  • Mentorship course

  • Evidence of ongoing professional development in relevant area of speciality supplemented by specialist clinical, managerial training and CPD
