Job summary

An exciting opportunity has arisen for a 4/12 fixed term contract for a dynamic and committed band 4 paediatric flow coordinator to join our team in the paediatric department providing support to the nurse in charge of their allocated wards in dealing with patient flow. They will work alongside the Multidisciplinary Team (MDT) to ensure that effective discharge planning and communication are maintained in order to provide a smooth, timely patient flow and discharge from hospital.

You will have experience as working with children ideally in a hospital setting. You will have a level 4 NVQ in Health and Social Care or other relevant qualification such as a diploma or equivalent experience.

You will need to be flexible in your approach to work the post will cover 7 days a week and will be from 09:00-21:00

Main duties of the job

The postholder will support the ward team specifically with:

Working alongside the designated medical team to prepare EDN

Supporting correct referral pathways into CCN, CHAHT team

Liaising with community and MDT colleagues regarding follow up (Specialist nurses, Health visitor, School Nurse)

Ensuring that everything is ready the day before discharge so that the patients can be discharged smoothly the next day.

Communicating with relatives on the day of discharge to confirm they understand process and have all relevant information to facilitate safe discharge.

Job responsibilities

Clinical responsibilities:

Lead on facilitating discharges in a timely manner within their allocated wards

Walk frequently across all allocated wards/areas to gain predicted/actual patient discharges, negotiate patient allocation, collate infection control status/updates and any relevant patient length of stay information.

Attend their allocated wards handover to update on key issues, identifying delays in the patients pathway which may cause blocking and where necessary escalating issues to the Nurse in charge/ Ward manager.

Working with the NIC/ Ward manager, plan and create capacity, adhering to Trust Infection Prevention and Control and bed escalation policies.

In collaboration with the NIC/ Ward manager ensure identified patients are safely stepped down to discharge areas ensuring sufficient capacity within the acute treatment areas.

Work autonomously to manage a cohort of patients in the discharge area, ensuring safety and care of patients and a timely safe discharge.

Ensure patient and families voice is captured by discharge surveys and patient feedback

Ensure all ward boards are up to date with safe staffing numbers

Liaise closely with colleagues to prioritise and plan own workload, reporting to the nurse in charge.

Work closely with the Integrated Discharge Nurses in ensuring that timely referrals are made, and any delays followed up. Understand the importance of progress chasing and escalating delays in patient discharges and/or flow.
