Job summary
The Frailty service at Dartford and Gravesham NHS Trust typically sees older patients over the age of 70, helping them with concerns that may be affecting their independence and well-being.
The post holder is responsible for providing a comprehensive administrative support to members of the Frailty team. To work unsupervised, prioritising work on a daily basis, undertaking the role flexibly and working closely with the Ageing and Health Lead Nurse, Ageing and Frailty Coordinator and Frailty Specialist Nurses.
The post holder will work within Evergreen Unit, where Outpatient Clinics take place. They will be responsible for designated clinics, the preparation and storage of notes, the reception and discharge of patients without supervision or direction.
Main duties of the job
Frailty Data Collection - Looking at the frailty Database making sure the data input related to Frailty team activity is reflected on the database and is accurate. This would include insuring that database field are complete and correct.
Booking FAU (Frailty Assessment Unit) patients onto PAS
Manage the frailty triage system ensuring referrals are received triaged and booked in a timely manner from all referral sources.
Clinic note management - Printing clinic notes and referral letters for patient attending FAU.
Ensure all EDN's have been completed by the Frailty team before notes are sent to scanning department.
Printing front sheets once patient have been seen and sending notes to scanning.
Manage Frailty Email inbox.
Manage the frailty Diary
Frailty Virtual Ward, ensure all patients who have been booked onto the Frailty Virtual ward are followed up and all drug cards are sent to the Hospital at Home team in the correct timely manner, notes are kept until the patient has been discharged from the Virtual ward.
