Job responsibilities
1. To ensure the highest standard of personalised nursing care is delivered to the children and their families in partnership with all members of the multidisciplinary team.
2. To have day to day responsibility for supervision of the assessment of care needs and the delivery and maintenance of optimal care outcomes in the Inpatient/outpatient setting, in relation to childhood diabetes
3. To provide specialist care and education to children and their familiesin Dartford, Gravesham and Swanley, both in the inpatient and outpatient setting
4. To provide professional leadership to the MDT in terms of diabetes care5. To be knowledgeable of the NMC code of conduct and to be accountable for practice in accordance with that code6. To support the Band 7 Paediatric Diabetes Nurse Specialist , Senior Nurse and Senior Sisters, and deputise in their absence
Specific Responsibilities1. Clinical responsibilities:1.1 Responsible for the assessment of the care needs of babies, children and adolescents, and develops programmes of care in relation to the needs of the child and family in relation to diabetes1.2 Responsible for the implementation of programmes of nursing care and evaluation of their effectiveness, ensuring that the care plan provides a continuous record of progress, response to treatment, rehabilitation and nursing care.1.3 Take responsibility for a defined caseload of diabetic patients within the inpatient/outpatient setting under the supervision of the Band 7 Paediatric Diabetes Nurse Specialist1.4 Work in partnership with children and their families in order to support and encourage involvement in their care, thus supporting family centred care1.5 Takes the lead in supporting parents, carers, siblings and other members of the childs support network as appropriate1.6 Responsible for, and takes the lead in setting and maintaining optimum standards of nursing care, ensuring that nursing guidelines and policies are adhered to within the inpatient/outpatient setting in relation to diabetes1.7 Work without direct supervision and be responsible for the management of his/her caseload within the hospital and outpatient setting under the supervision of the Band 7 Paediatric Diabetes Nurse Specialist1.8 Maintain effective communication with multidisciplinary team members, other hospital nursing and medical staff, general practitioners, community nursing staff and statutory and non statutory organisations that may be involved with the care of the child and family1.9 Attend appropriate meetings to include case conferences, multi disciplinary meetings and ward rounds1.10 To run Nurse Led Clinics for children, ensuring compliance to guidelines set out in National Service Frameworks1.11 Is aware of the spiritual, psychological and emotional needs of the child and their family and endeavours to meet these at all times1.12 Ensures seamless transition of care from the hospital to home setting1.13 Ensures smooth transition of care for children with chronic illness to young adult services1.14 Acts as a source of specialised knowledge and clinical expertise in relation to diabetes, to ensure care delivered to a child and family is appropriate, optimum and evidence based.1.15 Advise on the supply and safe use of clinical equipment to children and their families within the hospital and community setting1.16 Responsible for, and co-ordinates the custody of all drugs by checking, ordering and administering them in line with Trust policy both in the hospital and community setting1.17 Responsible for the location of emergency and resuscitation equipment and is aware of procedures both in the hospital and outpatient setting2. Managerial responsibilities:2.1 Reports to the Band 7 Paediatric Diabetes Nurse Specialist, Senior Nurse, Senior Sister or General Manager any concerns in relation to patient care, the environment, staffing or any thing that may affect the service as a whole to children and their families either in hospital or the community setting2.2 Reports any accidents or incidents involving children, relatives and staff to the Band 7 Paediatric Diabetes Nurse Specialist, Senior Nurse, Senior Sister or General Manager. Completes statements and incident form, reporting such incidents appropriately, undertaking investigations as required2.3 Manages all aspects of a given caseload within the hospital and outpatient setting, including discharge planning, under the supervision of the Band 7 Paediatric Diabetes Nurse Specialist2.4 Be responsible for the assessment of the most appropriate cost effective nursing treatment and equipment for children within the inpatient and outpatient setting2.5 Maintains accurate staff records in relation to study leave, annual leave, sickness, maternity leave, special leave and other meetings as they occur2.6 Reports any concerns to the Band 7 Paediatric Diabetes Nurse Specialist, Senior Nurse, Senior Sister, General Manager or doctor in relation to child protection. Liases with Named Nurse, Named Doctor or other professionals as appropriate. Responsible for ensuring that they attend child protection training annually2.7 Responsible for, and co-ordinates the provision of care within the community setting in relation to diabetes, ensuring that there is effective communication and liaison with members of the multi disciplinary team and other services as required2.8 Exercises leadership within the inpatient/outpatient setting, and supervises and co-ordinates the work of the junior team as required2.9 Acts up for the Band 7 Paediatric Diabetes Nurse Specialist in their absence to provide advice, expertise and management advice for Willow, Paediatric Outpatients in relation to diabetes.2.10 Undertakes duties on different paediatric wards or departments as required by service demands, within the 24hr period2.11 Takes part in organising the off duty rota, ensuring that there is sufficient cover in order to meet the needs of all children in relation to diabetes care and expertise2.12 Prioritise own workload within the context of the inpatient/ outpatient setting2.13 Responsible for the ordering of adequate supplies of stores in relation to diabetes care, having knowledge of budgeting costs in order that all resources are used appropriately and judiciously2.14 Responsible for ensuring that all nursing staff are professionally competent in the performance of their duties, and that they receive relevant training and assessment before being asked to perform those duties2.15 Responsible for ensuring that all staff are aware of their responsibility in the prevention and control of infection, liasing with the Senior Nurse and members of the infection control team as required2.16 Be aware of all relevant policies of the Acute Trust, implement and abide by them as appropriate3. Professional responsibilities:3.1 Organises, gives assistance to and mentors newly appointed staff, as part of their orientation process3.2 Acts as an effective role model to all staff, students, children and their families3.3 Provide professional leadership, critically evaluating current practices, developing innovative ideas to improve effectiveness and efficiency of care by collaboratively setting and measuring standards of care delivery3.4 Promotes and maintains a professional appearance and manner at all times in accordance with Trust policy3.5 Promotes the Department and the Trust as a whole, in the most positive manner to the children, parents and local population at all times3.6 Responsible for the co-ordination of accurate documentation and information systems both manually and by using the Departments computer systems3.7 Maintains and promotes confidentiality at all times ensuring that information is only given out with appropriate permission3.8 Responsible for co-ordinating and actively maintaining a safe environment for children and families, visitors and other staff, as required by the Health and Safety at Work Act, Health and Safety policy, Risk Management policy and COSHH regulations3.9 Acknowledges and abides by the Code of Professional Conduct for Nurses and is guided by advisory documents issued by the NMC3.10 Maintains and develops own professional practice, keeping abreast of new developments and concepts, both clinical and professional3.11 Act as a resource and educator for health professionals both within and outside the Trust, giving clinical guidance, advice and support in relation to the child with diabetes3.12 Maintains good relationships with staff organisations and voluntary bodies
