Job summary

An exciting opportunity has arisen for a Cardiac Rehabilitation Nurse or experienced Cardiac Nurse to join our friendly Cardiac Rehabilitation department at Darent Valley Hospital.

The post holder will use their specialist knowledge and skills within the team to deliver a highly effective and efficient service to the cardiac patients within the Dartford, Gravesham and Swanley area.

Our Cardiac Rehabilitation department executes all aspects of Cardiac Rehabilitation in line with BACPR/NACR, including Phases I-IV, individual nurse-led clinics and a comprehensive hospital based education/exercise programme.

This exciting role will provide the successful candidate with an opportunity to apply specific cardiac skills as well as to learn and develop further in the role.

Main duties of the job

The post holder is responsible for the implementation, management and development of all aspects of the Cardiac Rehabilitation service from Phases I-IV.

They will manage the CR service, promoting excellent standards of care, risk-stratify all Cardiac Rehab referrals and identify the appropriate pathways of care. They will ensure seamless transition of care from hospital to discharge, provide accurate information and support to patients and their relatives/carers of all aspects of their post-cardiac event recovery.

The post holder will independently manage telephone/1-1 nurse-led clinics, co-ordinate and continue to develop the hospital based Phase III programme, including exercise prescription. They will maintain the audit and data collection to continue to enhance the forward thinking service in line with NACR/BACPR.

They will be proactive in continued service development to enhance the patient experience, refer to other appropriate disciplines regarding aspects of ongoing care i.e. GP, Phase IV, dietician, ED clinic etc. and provide education to hospital based nursing, medical, AHP staff and GP's.
