Job summary

This is a fixed term full time post based predominantly in Basingstoke, although with the ability to work cross site and provide cross cover to both Andover and Winchester to ensure service needs are met. The post includes working in the HHFT Community Palliative care team which is based in St Michael's Hospice within the grounds of Basingstoke Hospital, supporting and working with the CNS, Hospice @ Home and therapy team to provide specialist palliative care to north and mid Hampshire. The post involves seeing people in their own homes, as outpatients and working alongside the primary care team in the community. It also offers opportunities to review patients in BNH and cover in the inpatient units.

The on-call commitment is 1 in 6 with 1 in 6 weekends covering Winchester Hospice inpatients and advice to the whole service remotely.

Main duties of the job

To work with the Consultants in Palliative Medicine to deliver high quality palliative care

  • Undertake medical assessment, diagnosis and management of patients in hospice/hospital/community settings
  • Initiate appropriate medical care and prescribe appropriately in line with HHFT policies and guidelines
  • Assess new referrals to the service and work with MDT to initiate appropriate investigations and treatment
  • Any other medical duties in emergency situations

Job description

Job responsibilities

  • Communication with patients relatives and carers regarding clinical condition and management plan. To be involved in family meetings as required
  • To work as part of the MDT and communicate clearly with colleagues. To attend weekly Specialist Palliative Care MDT meetings
  • Communicate unexpected changes in patients condition to the Consultant in charge and, when required, to on call medical staff
  • Record medical assessment and examination on electronic patient database
  • Review and update drug records, working with the Pharmacist; to maintain accurate and legible drug charts
  • Completion of TTO prescriptions and patient-held drug record cards when working in the in-patient setting
  • Completion of death certificates and cremation certificates, and liaison with Medical Examiners Office as required
  • Communicate with referring team and other healthcare professionals e.g. GPs, to ensure continuity of care when appropriate and attend community GSF meetings
  • To support clinical nurse specialists and junior doctors working within the team
  • To provide support to the community, Hospice @ Home and Homecare teams across the service
  • Provide advice and teaching to HHFT staff and members of the MDT
  • Participate in education events, including Trust Induction programme, junior doctor and medical student teaching, and teaching events for nursing staff, health care assistants and allied health professionals
