Job summary

As a Trust we are seeking to appoint an enthusiastic and motivated full time Consultant in Geriatric Medicine to join our team to support the development of the Elderly care and Frailty service.

Our trust is currently developing its front door services specifically with older people in mind. We currently run a non-medical frailty intervention team, (FIT) working across the week, across both sites, Basingstoke and Winchester to aid admission avoidance and increase timely discharges. The trust is now developing its same day emergency care pathways and short stay unit for older people and we require Geriatricians to be part of this exciting challenge.

We recognise the importance of creating a service for our older population across the whole healthcare system of Hampshire. Hence our service provision thus far includes a dedicated clinical communications line for frailty medicine, Telemedicine, which is digital health support for the NH/RH population and virtual wards for frailty. We run rapid access outpatient clinics on a daily basis for older patients presenting with frailty syndromes and are responsible for two dedicated inpatient elderly care wards on the RHCH site and 3 wards on the BNHH site. The wards run daily board rounds which are supported by Clinical Matrons and allied health professionals to provide a multidisciplinary team approach to patient care. This includes an older persons' mental health service.

Main duties of the job

We are looking for applicants with a wide experience in Geriatric Medicine who also have excellent team-working skills with the abilities and professional skills to lead service developments in the Trust. We are keen to promote education and learning and provide a good educational experience to trainees. We would welcome those with teaching skills and help support applications for honorary teaching posts at our associated medical school.

The successful candidate will lead a team of junior doctors and multi-disciplinary staff including Physician Associates in providing comprehensive medical care for their inpatients, clinic patients and emergency admissions. As part of a multidisciplinary team you will be committed to delivering clinical and quality targets agreed both nationally and locally with the clinical teams and commissioners.

We support flexible working patterns where that may be required however, additional commitments are as follows; On-call commitments are on the acute medicine rota. They currently involve a medical admitting take approximately 1:14. (Additional 2Pas for on-call duties). There is a weekend ward rota approximately 1:12 for morning ward round seeing targeted patients on the medical wards with time off in lieu. This might be negotiable for applicants who are not currently practising GIM.
