Frailty Specialist Clinician - Hampshire Hospitals NHS Foundation Trust
Location: Basingstoke & North Hampshire Hospital - Emergency Department and Acute Assessment Unit
Role Description:
The successful candidate will work within the Frailty Team in the fast-paced Emergency Department, Acute Assessment Unit and South-Central Ambulance Falls/Frailty Car project. Frailty Specialists are the role models of exceptional, appropriate and compassionate care of older people living with frailty.
The post is open to Allied Health or Nursing professionals who can demonstrate the skills to assess a patient's physical, cognitive and social ability to remain at home or put plans in place to meet any acute or long term needs.
Main duties of the job
Working in frailty requires and empowers the professional to work beyond the boundaries of their traditional role to provide assessment and personalised treatment and care planning which is truly patient centred.
The frailty team work very closely with our community partners and are actively involved in the Hampshire Together programme. We expect that this role will work flexibly across hospital, community and ambulance services in line with the evolving integrated care model strategy.

- Excellent communication skills
- Previous experience with older adults or those with complex needs

- A willingness and ability to work "hands-on" in all aspects of patient assessment and care
- Completed or looking to complete post-graduate education related to Frailty

The Frailty Team consists of specialist nursing, therapy and medical professionals who work together to provide Comprehensive Geriatric Assessment and personalised treatment and care planning to older people living with frailty when they initially present to the hospital. We aim to facilitate same day discharges utilising a discharge to assess model where possible or facilitate a tailored and efficient admission. Our assessment process promotes a holistic approach to patient care and aids communication with other services both in the hospital and the community. Our proactive hands-on approach to working at the front door is critical to achieving the right outcome for our patients.
We are an expanding service with aims to strengthen links with the community to provide high quality care to patients beyond our doors. We are also developing our work with the downstream wards in the hospital for those patients who require a longer admission to hospital. Our long term aim is to develop a frailty friendly Emergency Department and Acute Assessment Unit and ensure patients living with frailty receive timely specialist assessment and treatment in the most appropriate setting.
The frailty team are also currently engaged in number of pilot projects including a Delirium pathway, Ambulance falls/frailty response car, same day emergency care for older people as well as internal quality improvement projects.
