An exciting opportunity has arisen within the Department of Older Persons Services at Great Western Hospital NHS Foundation. We are looking for a qualified ACP or a Registered Nurse / AHP who is keen undertake the ACP training role over 3 years which would start at Band 7 Agenda for Change progressing to Band 8a following successful completion. We are looking for someone who is dynamic and enthusiastic with a passion to provide high quality care to meet individual needs of the older patient.
Older Person ACP representation is required to support the transformational development of the delivery of a comprehensive multi-disciplinary service that is holistic in it's approach and evidence based.
The successful applicant will work closely with the Matron, Head of service and Clinical Lead to proactively manage older adults in the acute setting.
Main duties of the job
This post is a highly specialist clinical practitioner post to lead the transformational development of the deliveryof a specialised Advanced Clinical Practitioner service for our older patients. This is a key role for the medicaldivision and we need a highly motivated individual with strong organisational skills to undertake this position.
The post holder will support the delivery of a comprehensive multi-disciplinary service that is holistic in itsapproach; evidence based and meets the individual needs of older patients.
The post holder must be able to clearly demonstrate specialist knowledge and skills at an advanced level. Theywill practice autonomously and also within a defined multidisciplinary team in accordance with local guidelinesand protocols. Responsibilities will include supporting the medical team with initial history taking, clinicalassessment, diagnosis, treatment and evaluation of care. The post holder will demonstrate critical thinking in thedecision making process with support from Consultants as required. They will deliver a professional approachto the provision of high quality nursing care, acting as a role model to junior staff. The post holder will work closelywith the Matron, Head of Service and Clinical lead to drive forward change within the service.

4. To work in partnership with the multi-disciplinary members of the older persons team to provide a Comprehensive Geriatric Assessment (CGA) for patients living with frailty and presenting in an acute setting.
5. Effectively lead and monitor the delivery of care to ensure the individual needs of the patients are met.
6. To recognise changes in patients conditions which require the intervention of others and refer on as appropriate.
7. To clearly present the patients point of view to others, including promoting and protecting the interest anddignity of the patients.
8. Provide information to patients, relatives and carers regarding the treatment options assessments, investigations and follow up.
9. To maintain accurate, appropriate and concise records, including documentation on electronic patient management systems.
10. Promote and lead the development, implementation and review of standards, policies, procedures andclinical guidelines.

- Significant post registration experience at Band 6 or above
