Job summary

To provide effective dietetic treatment for a delegated caseload of adult or paediatric in-patients or out-patients at the Great Western Hospital. To be responsible for the assessment, planning, delivery and evaluation of evidence based individualised patient care within the boundaries of the role. The post holder will contribute to the continuing development of standards of care. The postholder will be expected to continue with their own professional development.

To oversee the delivery of a specific area of the dietetic service and provide support and supervision to junior colleagues working in the service as required.

To act as a source of expertise for dietetic advice related to an agreed area of dietetics to patients, carers, medical staff, other healthcare professionals, GPs and the dietetic department.

Main duties of the job

1. To provide an in and or outpatient dietetic service for patients meeting the criteria for the dietetic service.

2. To undertake the specialist, holistic assessment of patients' nutritional status, for patients with more complex needs for example those with a complex presentation or communication challenges, using investigative, communication and analytical skills, taking account of medical, social, cultural and psychological states across all clinical settings.

3. To act as an expert practitioner formulating individualised management plans, using clinical reasoning/reflection and utilising a wide range of treatment skills. Nutritional treatment plans will be formulated to support ill health, rehabilitation and recovery.

4. To provide appropriate dietary advice to a range of patients with a variety of conditions for treatment, symptom control and/or nutritional support.

5. To use behaviour change techniques including active listening, motivational and negotiating skills to support patients with difficult dietary and lifestyle changes as well as helping them to recognise and overcome any barriers to these changes. It may be necessary to adapt consultation style and patient information to suit the individual needs. It may be necessary to convey complex information via an interpreter when the patient does not understand English or if the patient is deaf.

2. To work with the relevant members of the multidisciplinary team (MDT) including consultants, clinical nurse specialists and other dietitians to develop a service and protocols for dietetic care of relevant groups of patients.

3. To implement and update nutrition protocols including nutritional screening and associated patient information using appropriate tools.

5. To be responsible for maintaining own competency to practice at a specialist level through CPD activities and learning and development opportunities. To maintain a portfolio which reflects personal development.

6. To act in accordance with dietetic professional standards when advising patients, carers and other health care professionals in the dietetic management of service users.

7. To work on a daily basis with highly sensitive and complex changing needs of patients in emotionally charged situations both within the hospital and outpatient settings. The post holder must maintain sensitivity to the emotional needs of patients and carers whilst managing within themselves and others the emotional consequences of working in this area.
