Community Sister/Charge Nurse - Band 6

An exciting opportunity has arisen within our community nursing team for experienced community or district nurses to join the team and work across our unique Integrated Acute, and Community Care service to help us make a difference to our patients . We have a flexible approach to working and offer a variety of flexible working opportunities that can be considered to fit around your family and lifestyle.

Main duties of the job

To provide effective caseload management and care to those patients over the age of 18 requiring nursing care in their own homes and other community settings working autonomously and independently. The post holder will provide holistic care, assessment of the new patient , clinical advice, treatment and support to those people who require additional support to remain in their own homes, particularly during the later stages of life, as well as providing care to those people who request urgent intervention to address clinical issues such as wound care and catheter management. The post holder will be required to work with the multidisciplinary team, gaining support and advice in the management of care. This post is subject to lone working so experience of working alone is desirable.

Job responsibilities

We provide a range of care and treatment to predominantly housebound patients who often have complex care needs or long term conditions that require treatment and support in wound care, catheter maintenance, palliative care, supporting people to die at home, insulin and diabetes care and a whole range of other health conditions.

The post holder will be responsible for effective caseload management and high quality care to those patients over the age of 18 requiring bespoke nursing care in their own homes and other community settings, often working autonomously and independently. You will provide holistic care, initial assessment of new patients, and will offer expert clinical advice, treatment and support to those people who require additional support to remain in their own homes, particularly during the later stages of life.

You will work collaboratively across the multidisciplinary team which includes Community Matrons, Virtual ward,Tissue Viability Nursing, Continence Specialist, Urgent community response team comprising, Paramedics, IV specialist team , therapists and our Phlebotomy Service to ensure a seamless patient care journey.

  • RN (Adult)
  • Evidence of continuing professional development relevant for Band 6 post/ ? willingness to work towards, for example ? Specialist Practitioner Qualification ? PACR or equivalent ' ? 'Long term conditions' ? Frailty

Desirable

  • Specialist Practitioner Qualification
  • Clinically relevant Post Grad modules

Experience

Essential

  • Systematic approach to assessment, planning and evaluation of needs led care.
  • Able to carry out activities of a fully functional community nurse including end of life care, competent in the use of Mckinley syringe drivers, the administration of medications to support people in the last stages of life, catheter management skills including male and supra pubic catheter management ,wound care management and administration of iv antibiotics
  • Use of an electronic patient record system (SystmOne)
  • Previous community nurse experience and lone working advantageous
  • Experience in supporting junior staff and students through supervision and opportunity to achieve learning outcomes
