Main duties of the job
To promote quality improvement in patient care within the Trust by acting as the lead clinical nurse specialist overseeing the Trust OPAT service to ensure safe and effective management of patients who are discharged on IV antibiotics.
To plan and deliver the Trust OPAT service, in liaison with the Lead Antibiotic Pharmacist, liaising with specialist clinical teams and community partners to meet key service delivery objectives.
To update the shared Trust/Community held database and in-house antibiotic database that holds patient information for use by all specialists involved in the care of a patient receiving OPAT. To use this system to provide regular reports of progress and monitor key service delivery objectives.
To work with commissioners in conjunction with the Lead Antibiotic Pharmacist to further streamline and co-ordinate the OPAT Service identifying areas for development and further resources that may be required.

- To lead on development, implementation, evaluation and the update of policies and protocols and associated documentation relating to the delivery of the OPAT service across both the hospital and community, liaising with the Lead Antibiotic Pharmacist as appropriate, including:

- Patients self administration training protocols
- Patient Management Plans
- Drug preparation and management monographs
- Relevant patient information
- To work in conjunction with specialist clinical teams across the trust and other health care providers to manage patients discharged on OPAT.
- To use specialist knowledge across a range of procedures to provide expert advice and use clinical pathways to ensure safe practice.
- Lead on the maintenance of a database to record OPAT patient care details, including patient demographics, infection and treatment details, secondary and primary care clinician details, management/monitoring plans and patient outcomes.
- To act as the OPAT Specialist Nurse providing and receiving information that may sometimes be highly complex, sensitive or contentious in nature and communicating this, when relevant to Clinicians/Partners such as
