Job summary
We are looking for an experienced Transformation Practitioner to join our team.
The Transformation Practitioner will be responsible for supporting the Pathways Transformation Manager and Project Leads across the organisation to identify and to maximise benefit realisation via effective and timely project methodology and deliverables across several allocated improvement projects and business areas. They will support the production of key documents as defined within the project governance process and maintenance of core project governance.
The Transformation Practitioner will provide direction and support to the Trust's project work identifying dependencies and helping to ensure a standardised project approach is adopted across the organisation. This will include the provision of support to the Head of T&I Hub and Transformation & Improvement Hub in supporting and reporting of Programme Boards; attend Trust wide meetings as required and provide timely and accurate reports. They will also be responsible for providing support to improvement programmes to help realise the benefits of the projects, and ensure key metrics are being delivered. There is a requirement to report the progress and performance of projects to the relevant board / designated governance reporting mechanism.
Main duties of the job
As part of the Transformation & Improvement Hub, this role will play a key role in supporting the roll out of the Trust-wide continuous quality improvement methodology, 'Improving Together'. All Transformation & Improvement Hub staff are trained in and responsible for supporting the Coach House effective roll out to Divisions and front-line teams.
Planning and Organisation
- Take delegated responsibility from Project Managers to plan and design projects that may impact on one or more operational teams across divisions and the organisation.
- Proactively monitor overall progress of projects and programmes, manage interdependencies, resolve issues and implement corrective action as appropriate.
- Define and develop business cases associated with the improvement programmes in line with Trust guidance.
- Ensure that the improvement governance framework is adhered to.
- Ensure the integrity of the allocated improvement programmes, focusing on internal consistency, and alignment with strategic objectives and interface with other projects.
