Job summary

The Diabetes Specialist Nurse will be a clinical specialist, providing clinical advice, specialist skills, knowledge and expertise to health care professionals and people with diabetes and their family / carers.

This role will be primarily focused to work within the diabetes nurse inpatient service, however experience related to other areas within diabetes nursing will be advantagious.

Main duties of the job

Provide a specialist nursing service to assess, plan, deliver and evaluate diabetes management with the person with diabetes, their family/carers. This will include support, advice and education in order to maximise the potential quality of life and independence at all ages.

Job description

Job responsibilities

  • Support the management of inpatients with diabetes. Advise and recommend initiation of and changes to treatment plans. Monitor the effect of treatment whilst an inpatient and support ward nurses to plan safely for discharge.
  • Advise staff and patients of action to take in specific crisis / emergencies i.e. hypoglycaemia, hyperglycaemia and sickness / infection.
  • Advise patients of action to take prior to undergoing examinations which require the patient to be nil by mouth or to omit their diabetes medications.
  • Support pre-conceptual care and management of treatment during pregnancy.
  • Initiation of Insulin therapy.
  • Provide education sessions for staff and people living with Diabetes.
  • Provide telephone advice as required to other health care professionals and people with diabetes or their family / carers.
  • Set up continuous blood glucose monitoring on patients and analyse results with them.
  • Support patients requiring continuous subcutaneous insulin infusion (CSII)
  • Be able to
  • Act as patients advocate assessing and managing the real and perceived unmet needs of the individual / family; working with the multi-disciplinary Diabetes Team.
  • Act as a resource providing specialist guidance and support to staff involved in diabetes care.
  • Promote a culture of expert patient by empowering people with diabetes to become experts in managing their own care.

Essential

  • RGN Level 1or equivalent Health Care Professional qualification.
  • Evidence of recent development.
  • Proven ability in teaching and presentation skills.

Desirable

  • Relevant post registration diabetes course eg "Care of the adult with Diabetes module"
  • Recognised Teaching qualification
  • Advanced communication skills course or counselling course
  • Independent Nurse Prescriber
  • Carbohydrate Counting training course.

Experience

Essential

  • Clinical diabetes experience at band 5 or above
  • Experience of working in an acute inpatient area.
  • Evidence of management/ leadership skills and autonomous practice
  • Evidence of continued professional development
  • Can demonstrate clinical experience in a variety of areas.
  • Practical experience of working with people with Type 1 and Type 2 diabetes and their families / carers.

Desirable

  • Insulin initiation teaching.
  • Experience of CSII Pump management.
  • Experience in independent diabetes clinic consultations.
