Job summary

Come and join the Nurse led Pre-Operative Assessment. This is an environment where you are a practitioner who has to think like a nurse, therapist, surgeon, and anesthetist whilst being a patient coordinator and advocate, Can you be part of the team that provides effective to all elective surgical patients across all specialties and co-ordinate pathways of care? You will assess the patient's health status, identifying any risks of surgery/ anaesthesia so that they can be minimised, eliminated or accommodated prior to surgery, according to agreed pathways.

We are looking to recruit a band 5 nurse to join our existing and well established pre -operative assessment unit.

The Pre op Assessment Unit provides pre-operative assessments for Orthopaedic, General Surgery, ENT, maxillo facial, urology and gynaecology surgery. The unit is currently open Monday to Friday from 07.00 - 19:30pm.

The successful candidate will be responsible for the pre-operative assessment of patients being prepared for a wide variety of surgical procedures. The successful candidate will provide a comprehensive nursing assessment and possible physical examination once competency achieved.

Main duties of the job

To provide effective Nurse led Pre-Operative Assessment toallelective surgical patients across all specialties and co-ordinate pathways of care. Utilisation of an electronic preassessment system to triage patients and assign them to the appropriate nurse for further review. Assess the patient's health status with the aim of identifying any risks of surgery /anesthesia so that they can be minimised, eliminated or accommodated prior to surgery, according to agreed care pathways.

Undertake clinical assessment and where competent, physical examination, for patients that are seen. To gain and be confident with clinical decision making related to surgical procedures and anaesthesia

Main Responsibilities and Duties:

  1. Participate in the delivery of care to ensure the individual needs of the patients are met
  2. Demonstrate an ability to assess the patients health status and follow agreed care pathways regarding the patients suitability for surgery or suitability to be added to the elective surgical waiting list, in accordance with Trust guidance and policies. This will include an electronic triage process as well as assessing patients direct from an outpatient setting.
  3. Demonstrate an understanding of the physical, social and psychological care of patients about to undergo surgery
  4. Be able to offer health promotion as required.
  5. Develop and update patient information literature in accordance with Trust guidelines
  6. Be proficient and competent in phlebotomy
  7. Be proficient and competent in recording ECGs
  8. Order Pre-Operative tests and Investigations according to NICE and Trust guidelines
  9. Have the ability and knowledge to work within all aspects of the pre-operative patient assessment pathway for all elective patients
  10. Co-ordinate the results of tests and investigations, liaising with GPs and taking action as required to reduce cancellations of surgery and improve patient health outcomes
  11. Promote evidence based clinical practice and assist the Senior Sister in performance improvement within Pre-assessment.
  12. Act as a Link Nurse resource for a specified clinical area and disseminate information to the rest of the team
  13. To liaise with the Bookings team to ensure smooth administration of all clinics and waiting lists.
  14. Undertake any clinical role in the absence of other colleagues from the department.
  15. Assist in the provision of support to patients and staff in the delivery of individual nursing care in accordance with the Trust policies and procedures.
  16. Provide clinical advice and support to junior staff within the multidisciplinary team.
  17. Monitor the delivery of care against the appropriate documentation.
  18. Promote and maintain a safe, welcoming and comfortable environment for the patients, carers, relatives, visitors and staff.
  19. Assist in the implementation and delivery of the Trust Carers strategy
  20. Ensure clinical governance encompasses continuous quality improvement that focuses on the fundamentals of care (Essence of Care)
  21. Assist the Senior Sister/Charge Nurse in developing service improvements
  22. Actively promotes and contributes to nurse led initiatives within the department.
  23. There is occasional requirement to exert moderate physical effort for several short periods during a shift whilst pushing wheelchairs, assisting patients onto examination couches. And repositioning patients
  24. Liaise with the Senior Sister/Matron to identify areas of change in practice and ensure these are achieved.
  25. Assist in the effective leadership within the clinical area
  26. Demonstrate up to date knowledge and understanding of National, Trust and Divisional issues and priorities, to facilitate clear leadership and direction.
  27. Encourage and support junior staff within their working practice, maintaining confidentiality.
  28. Work closely with the Senior Sister/Matron/Clinicians to assist in implementing the Trust Clinical Governance and Nursing Strategy and work towards the development of the Unit
  29. Demonstrate effective communication with all staff ensuring dissemination and exchange of information.
  30. In the absence of a senior member of the team you will undertake the day to day management role of the Unit, with support from the Matron.
  31. Responsible for maintaining own clinical competence and professional development

    5. Regular VDU usage and the use of several software applications
