Job summary
We are an integrated, hard-working but highly rewarding multi-disciplinary team, who aim to make a real difference to people's lives. We are looking for an experienced, flexible and enthusiastic Physiotherapist with previous NHS experience who would relish the opportunity to join our well established inter-disciplinary team and have a desire to develop their skills in promoting health, well-being, independence and rapid assessment skills (rehabilitation, reablement, community discharge to assess beds, neurology, falls prevention and the treatment and management of long-term conditions) across seven days. There are currently exciting developments within the team.
You will need to be a car driver and owner - independent means of travel is essential for this role.
Main duties of the job
- To be professionally and legally accountable for all aspects of own work, including the management of patients/clients on your caseload.
- Use highly developed specialist skills to screen and prioritise referrals for home based and venue based rehabilitation services and refer to other services when necessary.
- Assess the impact of the home environment and patient/client's social network which may influence the treatment plan and present a risk to the personal safety of the therapist.
There is the opportunity to rotate within the services that we offer as a community rehab team including reablement and Homefirst, community discharge to assess beds, Parkinson's Disease, Specialist falls and vestibular service, homebased rehabilitation service and Urgent (Rapid) Community response.
Some of these are clinic based service however majority of these patients are seen at home within the Local Swindon area covering approx 12 miles end to end.
There are training and development opportunities within the team to support your personal professional goals. We work closely with the GWH hospital to facilitate hospital discharges as we are the same trust.
You will be joining a very friendly, social and supportive team with a wealth of experience in a variety of areas which the team is happy to share.
