An exciting opportunity has come up on Teal ward at the Great Western Hospital, Swindon.
Teal is a 36 bedded unit which compromises of a short stay area for the elderly community and an in-patient ward.
The ward has recently undergone a lot of new changes with arising developmental opportunities for all staff within the team. The training is available to all staff through the in-house training provider, the academy.
Teal and TOPSSU (Teal's older persons short stay unit) works closely within a large in hospital multidisciplinary team to plan discharges and long-term care with support agencies within the community setting.
TOPSSU has recently undergone a development which sees an increase in patient flow throughout our unit and for the trust as a whole. This exciting time enables our patients to be treated in the right place at the right time for their needs.

The post holder will be responsible for the delivery of high quality care to a defined patient group either within the acute hospital or community based setting. They are responsible for the assessment, planning, deliveryand evaluation of evidence based individualised patient care within the boundaries of their role. The post holder will contribute to the continuing development of standards of care including developments of nursingstandards and monitoring of outcomes. The post holder will be expected to continue with their own professional development.
