Are you ready to be part of the systematic anti-cancer therapy (SACT) clinical team to work in partnership with our medical professions, specialist nurses and other disciplinaries across the integrated care partnership (ICP) and integrated care system (ICS)?
We are looking for an experienced cancer pharmacist to be responsible for driving the development and delivery of SACT services at GWH and to our integrated community care partners. You will join the SACT clinical team to undertake the following:
- Carry out toxicity review post treatment
- Provide pharmacotherapy advice
- Undertake medicines-review for patients receiving IV/Oral SACT
- Educate and counsel patients, relatives/carers on oral SACT / supportive drugs
- Communicate and build SACT network with ICS partners and GPs
- Familiarise red flag symptoms to triage patients
- Develop integrated care pathways for SACT patients between secondary care and ICS
Main duties of the job
The post holder will work under the supervision of the Lead Pharmacist in Cancer Services to provide operational leadership for the SACT delivery at GWH and ICS. Other duties include working as part of the cancer pharmacy team to verify chemotherapy prescriptions and delivery of clinical pharmaceutical services to inpatients, outpatients and day cases.
Essential expertise and experience:
- Considerable experience in oncology/haematology
- Experience of undertaking complex patient consultation
- Strong interpersonal and leadership skills
- Drive for excellence and innovative working
- Team Player but can also work independently
- Staff supervisory experience
- Desirable: independent prescriber, PgDip Clinical oncology or equivalent
