We are looking for an exceptional, registered (adult / mental health) nurse with a passion for, and significant experience of, providing person-centered and relationship-centered dementia care. Combining direct work with patients and families with supporting the best practice of health and social care professionals, you will call upon your impressive knowledge of evidence-based bio-psychosocial interventions to improve the experience of carers / families affected by dementia.

Main duties of the job
The primary focus of the Admiral Nurse role is to integrate and improve dementia care for patients, carers/families affected by dementia across the acute-community pathway by providing clinical leadership on the delivery of person-centered and relationship-centered dementia care within operational areas of The Great Western Hospitals NHS Foundation Trust.
The Admiral Nurse (Acute) will improve the experience of families affected by dementia during an acute stay, work in partnership with the Admiral Nurse (Community Outreach) to manage care transitions for people living with dementia registered with a Swindon GP, and support effective discharge for people living with dementia and their carers/families in order to contribute to a reduction in frequency of admissions and length of stay.
The successful applicant will be employed by the Trust and based within the Department of Older Persons services (DOPS) at The Great Western Hospital. There will be the opportunity to gain experience of working both within the community and Great Western Hospital.
You'll become part of a network of Admiral Nurses, and will have the opportunity to access relevant post-graduate courses and develop professionally, informed by the Admiral Nurse Competency Framework.

The Admiral Nurse (Band 7) will focus on leading and delivering person-centered dementia care and optimising the outcomes for people with dementia and their carers admitted to The Great Western Hospital working alongside the existing Admiral Nurse who is providing Acute care and outreach in to the community. Both Admiral Nurses will work together in an integrated way across the care pathway to ensure that transitions of care are well managed, discharge is supportive and effective and carers/families feel better able to carry on in their caring role.

Main Duties & Responsibilities:
Clinical
- Advise on the care of people with dementia and their carers/families with complex care needs from peri-diagnosis to post bereavement support.
- Provide specialist nursing assessment, using the Admiral Nurse Assessment Framework, for carers/families affected by dementia during hospital admission.
- Improve identification and management of co morbidities and frailty for people living with dementia and their carers/families.
- Work collaboratively with the Admiral Nurse (Community Outreach) and other health and social care professionals, adhering to the principles of continuity and consistency of care, and share relevant information in a timely manner across healthcare boundaries to support high quality, personalised care.
- In partnership with the Admiral Nurse (Community Outreach), manage care transitions and support effective discharge for people with dementia registered with a Swindon GP and their carers/families in order to reduce frequency of admissions and length of stay.
- Develop and support person-centered care plans for people living with dementia including positive risk management (e.g. falls, delirium, depression, incontinence).
- Deliver relationship and family-centered approaches to care, which meet the needs of people with dementia, carers/families and staff.
- Provide a range of psychosocial interventions and emotional support to promote health and wellbeing for people with dementia and their carers/families.
- Support people with dementia, carers/families and staff to understand and respond to changes in behaviour and relationships as a result of dementia. Develop their skills and competencies in coping with the challenges and difficulties they may face (in the present and the future).

Education and Leadership
- In partnership with the Lead for dementia training, provide innovative dementia training and education to staff within the Trust at all levels, to improve staff attitudes, knowledge, skills and confidence, in line with best practice and guidance.
- Participate in and/or lead formal training events promoting best practice in dementia care and disseminating Admiral Nursing work.
- Provide leadership on the delivery of evidence based best practice in dementia care.
- Act as clinical role model to others, including student nurses, and mentoring other staff as required.
- Support the development and delivery of work streams as part of the Trusts Dementia Strategy.
- Provide professional and clinical leadership, mentorship and clinical expertise to the staff in the departments caring for people with dementia and their carers/families within Great Western Hospitals NHS Foundation Trust.
