We are looking for enthusiastic and highly motivated Podiatrist with significant clinical and experience in managing diabetic foot care.

This is an exciting development opportunity to work across the integrated diabetic foot care pathway and you will work collaboratively with colleagues from both the acute and community teams.

This post is an opportunity to make a difference to people with diabetic foot complications and to work within a multidisciplinary team under the mentorship of the diabetic foot coordinator.

Our client base is primarily high-risk and you will be responsible for implementing and coordinating the treatment and management of foot complications associated with this high risk caseload, outreaching into patients homes and working within the acute and community setting.

This post is within the integrated podiatry team, where you will work closely as part of the diabetes Multidisciplinary Foot Team (MDFT).

Main duties of the job

Your role is to assess and triage referred patients to aid the clinical decision making within the team and support admission avoidance.

This outreach post is designed to support those who find it difficult to travel into the acute secondary care setting. You will need to work in partnership with a variety of health care providers within primary, community and acute care across Swindon and North Wiltshire in a variety of different clinical settings

You will be able to work as an independent practitioner, confident in the advanced assessment and treatment of patients with highly complex foot conditions and as one of the clinicians in the integrated team. You will work within the secondary and community care setting to ensure at all times patients are treated with care and compassion.

You will work closely with the Diabetic foot coordinator and support the Podiatry Head of service in the development and delivery of integrated specialist (diabetic) foot care in BSW CCG area.

  • BSc (hons) Degree /Diploma Podiatry or equivalent qualification
  • HCPC Registered
  • Postgraduate study at or equivalent to Master Level in a relevant area of high risk foot or diabetes
  • Certificate in local analgesia
  • Evidence of ongoing professional development.
  • Independent prescriber or willing to participate in development

Desirable

  • Membership of a professional body and / or special interest group

Experience

Essential

  • Extensive evidence of managing clinical a caseload autonomously
  • Evidence of broad clinical knowledge of the management of patients with active and high risk foot problems ,particularly those with diabetes including assessment ,diagnosis and treatment ,
  • Good understanding and experience of working in a of clinical governance framework, including audit and risk
  • Experience of designing and delivering education and training events for patients and colleagues.
  • Experience of team management
  • Experience of working in a multidisciplinary team.

  • Experience of working in a community setting
  • Experience of developing new care pathways /protocols
  • Experience of service redesign
  • Change management experience, influencing others and delivering results.
