Job summary

Part time position - 08:00 - 18:00 Monday Only

To provide proactive and responsive practical and administrative support to ensure the efficient booking of transport for non-emergency patients.

To supervise the running of day to day Patient Transport and be EEAST's on site lead managing and prioritising demand during busy periods, whilst complying with the local Acute Trusts expectations.

To focus on operationally supporting the Hospital to expedite patients home through patient transport.

The post holder will be expected to efficiently liaise with internal teams at the hospital and the Non-Emergency Control room.

Main duties of the job

The role is to provide proactive and responsive practical and administrative support to ensure the efficient booking of transport for non-emergency patients travelling to/from healthcare settings.

To supervise the running of day to day discharges and be EEAST's on site lead managing and prioritising demand during busy periods, whilst complying with Hospital Site expectations.

To focus on operationally supporting the Hospital to expedite patients home through patient transport.

The post holder will be expected to efficiently liaise with internal teams at Hospital and the Non-Emergency Control room.

Alert appropriate colleagues within both the hospital and EEAST organisation to anticipated difficulties relating to transport for patients being discharged or transferred and assist in resolution of the same.

You will work along side the hospital and control using your knowledge and assessments to get the best results for discharges and transfers

The post holder will be an integrated member of the transport team, maintaining the crucial link between ED, Acute Care Units, wards, clinics and the site team.

Job responsibilities

Communication & Relationship Skills

  • Attending bed meetings and taking ownership of any transport issues or actions required, updating PCH staff on accurate numbers for transport booked that day, liaising directly with site management to e.
  • Participating in red to green weeks hosted by PCH Site teams
  • Attend monthly Operational meetings chaired by the CCG and collate and present any specific recurring issues as well as owning and carrying out appropriate actions from those meetings.
  • To communicate varied and complex information clearly, both in writing and verbally to ensure decisions and rationale on which they are based is clearly understood. This will include regular and routine reports to the appropriate forums.
  • To provide on a regular basis capacity information to the relevant senior management
  • Liaising directly with patients, reassuring those who become anxious, negotiating language barriers, being empathetic towards individual patient needs.
  • Coordinating hospital staff, communications centres, transport offices and ambulance crews to ensure patients are conveyed in the most effective and efficient manner, and the service level agreement is maintained at all times.
  • EEAST first point of contact for patients, representing the Trust as one of the few EEAST colleagues based in the hospitals.
  • Being a liaison point for the hospital teams to allow escalation of issues that inhibit transfer
  • Maintain effective and co-operative working relationships with all staff to support effective co-ordination of patient transfers
  • Use own initiative when dealing with patient queries, understanding that you may occasionally be exposed to confused, distressed patients/relatives, escalating complaints and incidents when necessary
  • To be a point of contact for crews who are delayed onwards, enforcing the abort and/or cancellation procedures where necessary.
