Senior Community Mental Health Nurse

Job summary

A Senior Community Mental Health Nurse works with older adults who suffer from functional mental health problems, and complexities of later life and are over 75 years of age and all age dementia. The role involves triage, duty responsibilities, assessment, intervention and care planning for these patients for a limited time on a team held caseload.

The Central Norfolk Intensive Older Persons Service (IOPS) is a specialist service that interfaces between secondary inpatient services and primary care. We across and liaise with community mental health teams, social services, GP practices, acute inpatient areas, mental health liaison services, nursing and residential homes, crisis response and home treatment and family/carers. IOPS is a crisis service that can offer alternative to inpatient care.

As a whole team we are keen to continue developing and to ensure that we are offering the best possible service to the service users we work with. We are looking for motivated people, with good interpersonal and communication skills, enthusiasm and drive.

We offer a 7-days per week service with the operating hours of 07:50 - 21:00.

There are currently 2 Senior Community Mental Health Nurse roles available within IOPS.

Main duties of the job

As a Senior Community Mental Health Nurse, you can expect to be:

Answering calls from service users and professionals in a duty worker role.

Triaging and assessing referrals for the service.

Working as a team in the assessment and treatment of service users

In person assessment of referrals.

Safety Planning and crisis management.

Undertake gatekeeping duties for inpatient admissions.

Documentation of the above, including referrals, risk assessments, care planning, continuation notes, letters and referrals to other teams.

Liaison with other professionals and teams.

Care planning and undertaking suitable interventions for service users.

Quality Improvement

You will also have the opportunity to co-coordinate shifts, provide clinical oversight and support to practitioners, supporting with teaching and training opportunities, leading on staff development and being a visible role model that echoes the values of the trust and the vision of the service.
