The team are based in Ipswich and you willbe working with one of the most complex demographics within Suffolk. We focus on Recovery based interventions and strive to work in partnershipwith those we care for, their families and their networks.We are looking for people to join us to continue to build on an already successful service.We aim to continuously improve the service delivery and outcomes as a part of NSFT.You'll be working alongside a multi-disciplinary team, undertake the role of the duty worker as part of our team's rota. You'll regularly be workingwith external agencies as part of a service users care, so being able to work well in a team and having excellent communication skills is vital. Youcan expect a thorough induction and training / preceptorship period, as well as opportunities for ongoing development.

Main duties of the job

You can expect a manageable case load and a supportive management team to ensure you're able to work independently but support is never faraway if needed.The typical job roles and responsibilities will include:o Working with other multi-disciplinary professionals within the team.o Undertake initial assessments.o Liaise with statutory and voluntary sectorso The successful candidate will be an integral part of this team. You will contribute to meetings both within the service and in widernetworks.o Effective personal planning and organisation of daily workload.o Flexible working and manage own working time effectively and accordingly to service demands.o Managing and providing a high quality of care.o Managing a case load of patient that have a range of mental health needs.o Initiate, create and maintain therapeutic relationships with patients and carers, ensuring that practice is patient centred.o Participating in the duty rotao Practicing in line with policy, guidance and legislation.Interviewer information packo Carrying out risk assessment, crisis and safety planning.o Working closely with the ACS mental health team to support our patients, meeting both their mental health needs and their CareAct needs.o To support patients in being discharged back to primary care.
