Job summary
This is a managerial post responsible for providing operational direction (under the guidance of the Patient Safety Manager), leadership, coordination of effective frameworks and advice to support the Trust's Patient Safety activities.
The post holder will be expected to be visible and accessible to provide support and guidance to health care professionals, managers, and staff in all aspects of patient safety and ensure the Trust's commitment to learning from a range of incidents in a coordinated, systematic, transparent, and focused way is achieved.
The post holder will be responsible for management of the Patient Safety function; including the Trust's Patient Safety Incidents requiring investigation process, Incident reporting process, Duty of Candour process, National Confidential Inquiry into Suicide, Local Real Time Suicide Surveillance, and Homicide and external reviews.
Main duties of the job
To support the delivery and maintenance of a range of organisational structures and processes related to patient safety. These include Patient Safety Incidents requiring investigation process, Incident reporting process, Duty of Candour process and National Confidential Inquiry into Suicide/ Local Real Time Suicide Surveillance and the ensuring senior leads for these areas of work are sighted on the relevant data, emerging themes captured in these processes, for strategic assurances and Trust learning and improving.
The role will provide advice and direction on matters related to Patient Safety Incidents requiring investigation process, Incident reporting process, Duty of Candour process and National Confidential Inquiry into Suicide and Local Real Time Suicide Surveillance.
The role will combine information from these structures alongside internal and external data to complete analysis and formation of judgements relating to these activities. This analysis will be presented to a range of stakeholders (internal and external) using a number of means such as verbal presentation and written reports.
The role will support the Patient Safety Manager and in their absence attend meetings to deliver reports and advise on patient safety issues.
To devise and deliver training in the respect of investigation methodologies, learning from incidents, deaths, and wider reviews both local and national to the organisation.
