Job summary
South Norfolk Older Persons Community Team supports people living in the South Norfolk and Breckland area. Our service supports people living with mental wellbeing alongside an assessment, diagnosis and treatment for people referred to the service from the GP or other health professional services who are concerned with the cognition and memory of an individual.
We are based at Gateway House in Wymondham, where we are able to invite people to clinic to support.
Following an assessment and diagnosis of dementia where possible, the assessor will make a recommendation to the GP to commence treatment. Our memory treatment team initiates, reviews and supports the individual and their carer(s) during the period of the medication trial, following a clear pathway whilst maintaining good communication with the GP until the care can be transferred back to the care of the GP. During this titration the nurse will be offering good post diagnostic support to support all parties with living with the diagnosis and referring onto other agencies where needs are identified. Following induction the successful applicant will be expected to work independently to completed both clinics and home visits. On going support and development is offered throughout the team and from consultants.
Some prior knowledge and understanding of dementia and treatment is preferred. A full induction and support will be provided to ensure quality and confidence in the process.
Main duties of the job
The successful applicant should be able to
- Show an enthusiasm, proactive and compassionate approach of supporting older persons living with poor mental health and / or symptoms of dementia.
- Manage an comprehensive assessment of a persons mental, physical and emotional wellbeing.
- Create and support a collaborative plan of care.
- Identify and manage risks positively
- Manage their own diary and be adaptive to change
- Use their professional judgement and initiative to support both patient and carers.
- Be proactive in the safe management of their own caseload
- Communicate effectively - using both discussions, formal letters and notes on our electronic clinical notes
- Be pro active in the recording and detailing of clinical notes.
- Be part of and supportive within a proactive team.
