We are looking for someone who is passionate, emphatic, resilient, with excellent communication skills both verbal and written and who enjoys being part of a Multi-Disciplinary Team.We have an exciting opportunity for you to join our Community Eating Disorders service in Norwich as a Care Co-ordinator.You should be motivated, have good interpersonal skills, personal resilience, and enthusiasm for working with people.Whilst it would be desirable for you to have experience working with eating difficulties, and/or young people and their families, we will support staff to access accredited training relating to the evidence-based models of care for those with eating disorders.
Main duties of the job
Building therapeutic relationships with people and their family/carers.
Manage a caseload of people with an eating disorder, focusing on their mental health and physical health needs.
Liaising with other health, social care and education professionals to ensure joined up care.
Maintain a high standard of clinical work in compliance with NICE guidelines for eating disorders and the Care Quality Commission.
Maintain accurate and clear documentation and offer clear communication.
Undertake Duty work on a roster basis, Triage new referrals into the service jointly with MDT, carry out assessments Jointly with MDT, co-produce care plans and risk assessments/contingency planning with young people and their families.
