Job summary

The Crisis Response and Home Treatment Team are looking to recruit compassionate, dedicated and conscientious individuals whowantto make a positive difference toadults experiencing mental distress and mental health crisis.

This role is to work across the crisis support pathway based predominantly at Hellesdon Hospital in Norwich. We can offer you the opportunity to rotate to the Mental Health Liaison Team based at our local acute hospital (NNUH) and hub working with our third sector partners that will be providing crisis support and housing. You will work within a multidisciplinary team providing care, treatment and assessment to patients both at base and in their own homes.

The CRHT team are very proactive and highly skilled at managing risk and supporting service users in South, North and Central Norfolk who present in crisis and persons needing intensive support to avoid hospital admission.

Main duties of the job

The team is the 'front-door' to acute services, they provide triage, assessment, intensive support and admission avoidance hand in hand with support of families and 3rd party providers. The staffing teams also works with service users already in hospital to ensure that they can be supported at home at the earliest opportunity to support their recovery.

There is an expectation thatyou will have the ability to clinically lead care delivery and will be proficient in clinical care including care/discharge planning, risk assessment, admission prevention, environmental health & safety, Mental Health Act work and physical health care.

We are looking for dynamic registered nurses / OT to who want to be involved in assessments and home treatment for people in crisis who are keen to be part of transformation and improvement in the area of crisis care. We want creativity, innovation and passion towards providing excellent patient care.
