Job summary

Community Mental Health Team for Older People in Norwich and North Norfolk.

Working with Older People who have mental health needs, including dementia. The team provide care for people with complex mental health needs and a memory assessment and treatment service.

Are you a Nurse who has experience of managing complex demands and can demonstrate being creative and caring? Our lively and supportive Community Mental Health for Older Adults Team can offer you an exciting opportunity to join them.

As a team we focus on our staffs' Continuing Professional Development and have a drive for further developing our staffs' clinical knowledge and skills. The team also have established links with Primary Care and other agencies, which enhance our work and ensure we are able to help and support our patients.

Main duties of the job

The post holder will undertake the planning and delivery of direct patient care with appropriate supervision, ensuring continuity of practice and working within a multi-disciplinary and multiagency environment.

  • Managing a caseload and the provision of Mental Health Care services to people with mental health care needs. Support and advising to other health care professionals. To support aspects of service development.
  • Provide an appropriate learning environment for relevant UEA students.
  • To be responsible for accurate mental health clustering of all patients on admission, discharge and at review. To ensure all mental health clustering information is passed onto other Trust teams or services on transfer of patients.
  • To ensure the accuracy and quality of mental health clustering activity within the team.

This is an exciting opportunity to relocate and work 9-5 hrs between Monday to Friday.
