You will undertake the planning and delivery of direct patient care with minimal supervision, ensuring continuity of practice and working within a multi-disciplinary and multi-agency environment. Managing a caseload and the provision of mental health care services to people with mental health care needs.

Main duties of the job
We are currently working in a more flexible manner with increased use of virtual mediums to conduct patient care, and therefore this will provide opportunity to discuss flexible ways of working. We work collaboratively and in a supportive manner which is of particular importance in these unusual times.
Main duties of the job:
- Managing a caseload of patients that have a range of mental health needs.
- Undertaking initial assessments
- Completing care plans in collaboration with the patient
- Signposting to others service
- Multi-disciplinary team meetings
- Inter-agency meetings
- Participating in the duty rota
- Participating in the depot clinic
- Making safeguarding referrals where necessary
- Working alongside our psychology team to offer psychological interventions to patients.
- Carrying out risk assessments, risk, crisis and safety planning.
- Build rapport with those difficult to engage.
- Support patients being discharged from an inpatient unit.
- Working alongside and in partnership with other NSFT teams.
- Working closely with the ACS mental health team to support our patients, meeting both their mental health needs and their Care Act needs.
- Supporting patients to reach recovery or their optimum level of mental health.
- To support patients in being discharged back to primary care.
